Letter from the Chief Inspector of General Practice

We carried out an announced comprehensive inspection at Ledbury Market Surgery on 24 November 2016. The overall rating for this service is good.

Our key findings across all the areas we inspected were as follows:

  • The practice was aware of and provided services according to the needs of their patient population.
  • Patients told us they were treated with dignity and respect and that they were fully involved in decisions about their care and treatment.
  • There were processes and procedures to keep patients safe. This included a system for reporting and recording significant events, keeping these under review and sharing learning where this occurred.
  • Staff received regular training and skill updates to ensure they had the appropriate skills, knowledge and experience to deliver effective care and treatment.
  • Regular meetings and discussions were held with staff and multi-disciplinary teams to ensure that patients received the best care and treatment in a coordinated way.
  • There was a clear leadership structure and staff told us they felt supported by management.
  • The practice was aware of the requirements of the duty of candour and systems were ensured compliance with this.
  • There was a culture of openness and accountability.
  • The practice had an active Patient Participation Group (PPG). The PPG was proactive in representing patients and assisting the practice in making improvements to the services provided.
  • Information about services and how to complain was available and easy to understand. Patients told us that they knew how to complain if they needed to.

During a check to make sure that the improvements required had been made

At our previous inspection on 17 October 2013 we found that improvements were needed to ensure that the provider had suitable procedures in place to deal with potential emergencies that might reasonably be expected to arise.

We recently asked the provider to send information to us to show that the required improvements had been made. We checked this evidence. We saw that changes had been made and medicines were now monitored more effectively, and emergency supplies were now available for the benefit of the patients who used the practice.

During a routine inspection

We spoke with six patients during our inspection. They were all extremely complimentary about the service they received at the surgery. One patient told us, 'It's the best practice I've been to'. Another said, 'It's fantastic'. Patients told us that they could generally get a routine appointment quite quickly. They also told us that they could always get an urgent appointment on the same day if they need one.

We saw that patients' views and experiences were taken into account in the way the service was provided. We also saw that the surgery had policies and procedures in place to help protect patients' privacy and dignity. The staff we spoke with were familiar with the procedures.

We found that care and treatment was planned and delivered in a way that met patients' needs and protected their rights. Patients were able to be involved in decisions about their treatment.

The surgery had effective safeguarding procedures in place and staff were familiar with them. We found that staff had received appropriate training for their role. We also found that staff were appropriately supervised and that they received regular performance appraisals.

There was an effective complaints system in place and the surgery responded to complaints appropriately.

We were not satisfied that the provider had suitable procedures in place to deal with emergencies that might reasonably be expected to arise.
