We looked for evidence that service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture. At our last assessment, we rated this key question as good. At this assessment, the rating remains the same.

Shared direction and culture

The service had a shared vision, strategy and culture which was, ‘To provide high quality primary care services through an ethos of continuous learning and reflection, whilst ensuring all staff and patients are treated with dignity, honesty and respect.’ Staff and leaders understood the challenges and the needs of people and their communities.


The development of the service’s vision and strategy was kept under review by leaders and staff were asked for their views on this. Leaders monitored and reviewed progress against the delivery of their strategy. To support strategic planning, a business development plan was in place. This included issues such as succession planning for future retirements, staffing requirements alongside the ongoing development of their multidisciplinary team, the revitalisation of the Patient Participation Group and expansion of the practice.


The service was aware of local challenges which affected their service, such as a high number of patients with long-term conditions, and worked with partner agencies to address these challenges. Staff and leaders actively promoted equality and diversity and worked to identify the causes of any workforce inequalities. They demonstrated a positive and compassionate listening culture that focused on learning and development. Staff were very positive about the culture within the practice and used words such as open, supportive, positive and non-hierarchical to describe it.

Governance, management and sustainability

Score: 3

The service had clear responsibilities, roles, systems of accountability and good governance. They used these to manage and deliver good quality, sustainable care, treatment and support. They acted on the best information about risk, performance and outcomes, and shared this securely with others when appropriate.


The practice’s registration with the Care Quality Commission needed to be updated to reflect changes within the partnership. Immediately following our assessment, the provider submitted the required checks and application forms to update these changes.


All staff we spoke with were clear on their individual roles and responsibilities. Leaders proactively supported staff and regularly met with them to complete appraisals and performance reviews. The provider had established governance processes in relation to the quality statements we reviewed during this assessment, which were appropriate for their service. Staff could access all required policies and procedures, which were kept up to date and reflected latest guidance. Leaders held ad hoc meetings with staff when issues were identified and discussed emerging risks and potential improvements to the service. The provider acknowledged these meetings needed to be held on a more regular basis to ensure a proactive approach and had updated their business development plan to reflect this. Staff took confidentiality and information security seriously, which included verifying people’s identity during remote consultations. Staff submitted data and notifications to external agencies as required.
