The provider had made improvements to systems and processes following our last assessment and is no longer in breach of regulations related to staffing and good governance.

Roles, responsibilities and accountabilities were now clearly known by staff.

Clinical governance had improved. The service now had an embedded programme of audit to improve the quality of care.

Leadership had effective oversight of risks and performance. Where this indicated improvements could be made, action was taken.

Governance processes overall had been improved. Management had clear oversight of all processes. We found recruitment processes ensured staff were recruited safely. Supervision and appraisal arrangements were embedded in the practice.

Governance, management and sustainability

Staff told us roles and responsibilities within the practice were now clear and continued to describe the leadership as approachable and supportive.

Care and treatment followed national guidance or evidence-based best practice. An embedded programme of audit now identified if this had not been followed. Action was taken to address shortfalls and improve the quality of care.

The practice now had governance processes to ensure areas of poor performance were identified and acted upon. Changes were made and resulted in improvement. For example, the practice no longer had a backlog of notes requiring summarisation and monitored the status of incoming correspondence to prevent a backlog recurring. Additional staff had also been identified to support this work.

Leaders had effective oversight of risks within the practice and embedded governance processes to manage these. For example, the practice had changed to a new training platform. There was a risk the compliance status of staff would not be accurately presented to management during the transition between the old and new systems resulting in staff potentially not having the knowledge to carry out their roles. However, management were aware of this and had processes to monitor and ensure compliance. Our reviews of training records found all staff had completed all the training required by the practice.

Processes to ensure confidentiality and integrity of data were in operation.

Communication between all roles in the practice was open and honest. Staff felt comfortable to raise concerns.

Succession plans existed to ensure continuity of staffing and there was a business continuity plan in place.
