The provider had made improvements to systems and processes following our last assessment and is no longer in breach of 2 regulations related to safe care and treatment and staffing.

The use of national guidance and evidence-based best practice for monitoring patients was now embedded in the practice.

Patients received care, treatment and support which optimised their health. Annual reviews of medicines and conditions were now structured and carried out routinely.

Patients with long-term conditions now received routine blood monitoring and prescribing of medicines was based on up to date and accurate information.

Patients’ needs were assessed, including communication and support needs.

Patients were involved in the assessment of their needs, and support was provided where needed to maximise their involvement.

Patients felt well supported with their care and that staff were kind and compassionate.

Assessing needs

Medicine reviews were now structured and blood monitoring followed the intervals in national guidance. This meant people’s care and treatment was routinely reviewed and care and treatment decisions could be made using accurate information.

The practice promoted the benefits of cervical screening to patients. Unverified data was shared which showed the practice had achieved an improvement in the uptake by patients on the previous year.

Staff we spoke with told us how they considered the needs of carers to ensure they managed their own health and wellbeing as well as the people they cared for.

Patient feedback from national surveys showed that people using the service felt appropriately involved with decisions about their care and treatment. We received a limited amount of feedback from patients which was positive about their care and also reviewed unverified data provided by the practice. This all supported the fact that patients were positive about their experience of receiving care and treatment from the practice.

Delivering evidence-based care and treatment

The practice had a programme of clinical audit which involved all the staff groups within the practice. This ensured staff were kept up to date with current guidance and best practice. The outcomes of audits were shared with stakeholders to support the wider healthcare system to improve care for patients.

The practice now had embedded systems and processes that ensured patients received structured annual reviews of their conditions and monitoring within timescales recommended in national guidance. Our remote clinical searches reviewed the care and treatment for patients with long-term conditions including diabetes, asthma, hypertension and chronic kidney disease. All the patients reviewed had received appropriate care to manage their conditions.
