We looked for evidence staff involved people in decisions about their care and treatment and provided them with advice and support. Supporting people to live healthier lives

The service always supported people to manage their health and wellbeing to fully maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support. For example, the service had implemented a robust social prescribing system, through which 661 patients had been signposted to appropriate support services over a 12-month period.

Staff focussed on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff actively supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity. For example, they operated patient education evenings covering multiple topics, facilitated early intervention therapy services via Youth Mind Matters and arranged support for patients in applying for some benefits.

Monitoring and improving outcomes

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves. The provider had recently undertaken a targeted review of patient records to identify emerging clinical trends and used this information to prioritise needs, identify required resources, and develop appropriate response mechanisms. As a result, a new appointment and triage system was implemented, and an additional GP was recruited to improve capacity and better meet patient demand.

 

The service met national targets for the uptake of childhood immunisations. The service performed better than the England average in relation to several cancer screening programmes.
