The service was fully aware of all potential risks in the care environment and controlled them well. They made sure equipment, facilities and technology supported the delivery of safe care.

The practice owned the building it occupied and were meticulous in maintaining it. They ensured that equipment, facilities, and technology supported the delivery of safe care. Required checks, including portable appliance testing (PAT), and equipment calibration, had all been completed in a timely manner. There was a clear programme to keep checks up to date.

Annual fire safety drills were undertaken and learning shared with staff.A recent fire evacuation had demonstrated the effectiveness of the training.

Effective systems were in place to track prescription stationery through out the practice.

The service had a comprehensive and up to date business continuity plan outlining how it would respond to disruption of the service. A hard copy was included in ‘grab bags’ in several locations around the practice which also contained other items useful during an emergency evacuation.

 

Safe and effective staffing

The service made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs.

The service employed a range of clinical and non-clinical roles, which included GPs,nurses and health care assistants. Leaders ensured staff were up to date with their training which the service had deemed mandatory and operated within their agreed areas of competence.

We saw evidence of good supervision of the practice’s physician assistant. The Total Triage system employed at the practice ensured that only appropriate cases were directed to them. We also saw records of supervision and advice sessions for other clinical staff in the practice where they had sought guidance on specific issues. The service followed safe recruitment procedures when employing staff, which were in line with national legislation. This included identity checks, review of qualifications, obtaining of professional references and a criminal records check.

Infection prevention and control

The service assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly.

Effective infection control processes were in placetomanage the risk of infectionand todetect and control the risk of it spreading.The service’s infection prevention and control (IPC) lead conducted regular risk assessments and audits to ensure compliance andactedwhere necessary to mitigate any identified risks.

All staff were trained in infection prevention and control to a level appropriate to their role.

The practice had comprehensive cleaning schedules in place and cleaning was carried out by an external company. The practicedemonstratedhow they audited the cleaning company’s performance to ensure compliance both with the schedule and cleaning policy.

During our onsite visit, the service's premises and a sample of equipment reviewed were found to be visibly clean.

There were comprehensive arrangements in place for the management of clinical waste and sharps. Sharps boxes were available where needed and were appropriately labelled.
