We looked for evidence that staff monitored people’s care and treatment and supported them to live healthier lives. At our last assessment, we rated this key question as good. At this assessment, the rating remains the same.

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

Staff were focussed and proactive on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including; tackling obesity, the service ran its own evening slimming group which had been going for over 10 years. The service utilised social prescribers reviewing the intervention to determine the impact this has had for the patient. The service had a large elderly population and had taken part in the aging well project for the last 6 months. Advanced Care Paramedics (ACP) did a clinical assessment with the patients, with a focus on chronic disease management.

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

The service met national targets for the uptake of childhood immunisations. The service performed well in relation to several cancer screening programmes, including screening for breast, bowel and cervical cancer, finding innovative ways to ensure patients respond to invites to come in for these tests.
