The service had a shared vision, strategy and culture. This was based on transparency, equity, equality, human rights, diversity, inclusion and engagement. Staff and leaders understood the challenges and the needs of people and their communities.
Leaders and staff had developed a shared vision and culture that placed patients at the centre of care delivery. Staff had contributed to the development of the service's vision and values, which were regularly reviewed to ensure they continued to reflect the needs of the practice and local population. Leaders told us the practice had recently revisited its mission statement with the whole team, encouraging staff involvement and ownership of the practice's objectives. They were committed to retaining the identity of a small, community-focused practice, while also embracing opportunities to develop and improve services.
Leaders described the practice as both traditional and forward-thinking, with a strong emphasis on providing continuity of care and maintaining the ethos of a ‘family doctor’ service. Leaders were also conscious of creating a positive and supportive working environment. They demonstrated flexibility in accommodating working hours where possible to support staff wellbeing and work-life balance. Staff told us GPs were approachable and accessible, which supported effective communication and collaborative working.
The practice actively engaged with patients through their Patient Participation Group (PPG), which met quarterly and contributed to discussions about service development and improvement. This supported the practice's commitment to working collaboratively with patients and fostering a culture of openness, engagement and continuous improvement.

Governance, management and sustainability
The service had clear responsibilities, roles, systems of accountability and good governance. They used these to manage and deliver good quality, sustainable care, treatment and support. They acted on the best information about risk, performance and outcomes, and shared this securely with others when appropriate.
The service had established governance arrangements to ensure compliance with best practice. Staff had access to up-to-date policies and procedures and demonstrated a clear understanding of their roles and responsibilities. The service maintained a flexible workforce, with staff routinely providing cover for colleagues during periods of annual leave, which supported continuity of care for patients. Leaders provided ongoing support to staff through regular appraisals and performance reviews and had implemented succession planning arrangements to address future workforce needs, including staff retirement. Regular team meetings were held to discuss emerging risks, service developments and opportunities for improvement. Actions arising from these discussions were clearly documented, monitored and communicated to staff.
The service worked collaboratively with the primary care network and other partner organisations to support service delivery and improve outcomes for people using the service.
