We looked for evidence that staff monitored people’s care and treatment and supported them to live healthier lives. At our last assessment, we rated this key question as good. At this assessment, the rating remains the same.

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

The practice ensured staff remained up to date with current guidance and best practice through regular communications and updates, enabling them to provide patients with appropriate advice, support and care based on the latest recommendations. The service had invested in an automatic recall system for patients with long-term conditions to encourage attendance for routine reviews and clinical monitoring. Staff were able to refer patients to a diabetes prevention programme that included dietary and lifestyle education and advice.

Staff focussed on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity. The social prescriber from the primary care network was utilised by the practice. They were able to provide a vast range of support from supporting people’s mental and physical health.

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

At the time of our assessment, the latest published information from the UK Health Security Agency (UKHSA) showed that the practice mostly achieved the World Health Organisation target of 95% for the number of children immunised against various infectious childhood diseases except for children aged 5 who had received immunisation for measles, mumps and rubella. However, due to the practice’s small patient population, a single child not receiving the vaccination had a significant impact on the overall percentage achievement. The practice assured us they were engaging with non-attenders.

The latest published information from NHS Digital showed the practice’s uptake for cervical cancer screening met the 80% national target for women aged 50 to 64 years old. However, it was observed at 78.2% for women aged 25 to 49 years old. The practice told us they had taken steps to increase the cervical screening uptake. For example, people received multiple invites and staff used opportunities during routine consultations to encourage eligible patients to attend and raise awareness of the importance of screening. Audits were completed to monitor and improve the quality of care.
