During an annual regulatory review

We reviewed the information available to us about Old Swan Health Centre Group Practice on 1 October 2019. We did not find evidence of significant changes to the quality of service being provided since the last inspection. As a result, we decided not to inspect the surgery at this time. We will continue to monitor this information about this service throughout the year and may inspect the surgery when we see evidence of potential changes.

Letter from the Chief Inspector of General Practice

We carried out an announced comprehensive inspection of this practice on 30 June 2016. A breach of legal requirements was found. After the comprehensive inspection, the practice wrote to us to say what they would do to meet legal requirements in relation to:

  • Regulation 19 HSCA (RA) Regulations 2014 Fit and proper persons employed.

We undertook this focused inspection to check that they had followed their plan and to confirm that they now met legal requirements. This report only covers our findings in relation to those requirements. The practice is now rated as good for providing safe services and good overall.

Our key findings were as follows:

  • The practice had addressed the issues identified during the previous inspection. Disclosure and Barring Service (DBS) checks had been completed for all staff.
  • In addition, the practice had purchased a defibrillator; put monitoring systems in place for uncollected prescriptions, the use of prescription pads, expiry dates for the contents of the first aid kit; and now had information available for carers and easy read/large print information for patients.

During a routine inspection

Letter from the Chief Inspector of General Practice

We carried out an announced comprehensive inspection at Old Swan Health Centre Group Practice on 30 June 2016. Overall the practice is rated as good.

Our key findings across all the areas we inspected were as follows:

  • The practice is situated in a purpose built health centre next to a walk in centre. The practice was clean and had good facilities including disabled access and translation services. Some GPs spoke various languages including Punjab, Cantonese and Mandarin.
  • There were systems in place to mitigate safety risks including analysing significant events and safeguarding.
  • Patients’ needs were assessed and care was planned and delivered in line with current legislation.
  • Patients said they were treated with compassion, dignity and respect and they were involved in their care and decisions about their treatment.
  • Information about services and how to complain was available. The practice sought patient views about improvements that could be made to the service; including having a patient participation group (PPG) and acted, where possible, on feedback.
  • Many of the staff had worked at the practice for a long time and knew the patients well. Staff worked well together as a team and all felt supported to carry out their roles.
  • The practice was aware of and had systems in place to ensure compliance with the requirements of the duty of candour. (The duty of candour is a set of specific legal requirements that providers of services must follow when things go wrong with care and treatment).

However, there were areas where the provider must make improvements.

The provider must:

  • Ensure appropriate recruitment checks are carried out for all their staff.

The provider should:

  • Have monitoring systems in place for the use of prescription pads.
  • Consider having a hearing loop and easy read format information available for patients.
  • Check staff are aware of the practice policy to monitor uncollected prescriptions and who has responsibility for this.
  • Consider having more information available for carers.
  • Regularly review the contents of the First Aid kit.
  • Have a written shared agreement with the adjoining walk in centre for the use of the defibrillator.

During a routine inspection

We spoke with a number of patients about their experiences of the practice and the feedback we received from each of them was very positive. One person commented; "It is a really good practice." Another person told us they felt the practice was, "Very good."

The practice had up to date child and adult protection policies and procedures in place. This information included contact details for staff to raise concerns with the appropriate agencies.

We saw records were kept of adverse events, accidents or incidents including actions taken. The practice completed audits/reports following significant events or receiving complaints, in order to learn and make improvements as required.
