Care provision, Integration and continuity
The service understood the diverse health and care needs of people and their local communities.
Providing Information
The service supplied appropriate, accurate and up-to-date information in formats that were tailored to individual needs.
Listening to and involving people
Staff involved people in decisions about their medicines and helped them understand how to manage their medicines safely. People were involved with assessments and reviews about managing their medicines safely and to make sure their preferences were included. People were told who to contact if their condition did not improve or they experienced any unexpected symptoms.
The service made it easy for people to share feedback and ideas, or raise complaints about their care, treatment and support.

Equity in access
The service made sure that people could access the care, support and treatment they needed when they needed it.
Equity in experiences and outcomes
Staff and leaders actively listened to information about people who were most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this.
Planning for the future
People were supported to choose appropriate treatments. For example, the options of the type of medicines available were discussed with patients.
