We looked for evidence staff involved people in decisions about their care and treatment and provided them with advice and support. Staff regularly reviewed people’s care and worked with other services to achieve this. At our last assessment, we rated this key question as good. At this assessment, the rating remains the same.

The surgery always supported people to manage their health and wellbeing to fully maximise their independence, choice and control. The surgery supported people to live healthier lives and where possible, reduce their future needs for care and support.

In the past 12 months, 100% of people with a severe mental illness that wanted support had received a health check. All eligible people with a learning disability were offered a health check, and 100% of those who wanted one received it.

Surgery leaders and staff supported national priorities and initiatives which included for example, stopping smoking and tackling obesity. A comprehensive range of information was seen and available in the waiting room area and on the surgery’s website which signposted people to information and local and national support groups to encourage healthier living.

Staff focused on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity. Leaders told us, ‘we aim to provide the best possible service to our patients to improve their health and wellbeing’. Leaders told us they work in partnership with their patients and local healthcare providers. This was confirmed with the people we spoke with during the onsite assessment and with the local care homes that the surgery visited weekly and provided a primary care service to. The care home staff representatives we spoke with told us the surgery was easy to access, if they needed an urgent visit this was always provided and prescriptions were managed effectively and in a timely manner.

The surgery monitored all people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and fully met the expectations of people themselves. This was confirmed by the people we spoke with at the surgery and care home staff we spoke with. The surgery shared the clinical audits they had undertaken showing the improvements and changes made to people’s treatment and prescribing to ensure improved clinical outcomes.

The surgery exceeded national targets for the uptake of childhood immunisations performing well above the World Health Organisation (WHO) 95% target. The surgery also performed better than the England average in relation to several cancer screening programmes, including screening for breast, bowel and cervical cancer.
