Supporting people to live healthier lives

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.
Staff were kept up to date with relevant guidance and best practice through regular communications and updates shared via the service's electronic management system. The service operated effective recall systems for patients with long term conditions, using text messages, telephone calls and letters to encourage attendance for reviews and monitoring.
The service also had processes to identify patients who were eligible for immunisations and vaccinations in line with national guidance. Regular searches were completed to identify patients’ due vaccinations, and reminder systems were used to support uptake, including for childhood immunisations and seasonal vaccination programmes.
 

Monitoring and improving outcomes

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

The service achieved national targets for childhood immunisations, with uptake rates exceeding the national minimum target of 90%. For example, 99.1% of children aged one had completed their primary immunisation course, exceeding the World Health Organisation (WHO) target of 95%.

Data showed that 80.7% of eligible women aged 25 to 49 and 82.9% of eligible women aged 50 to 64 had received appropriate cervical screening, both above the national target of 80%.
The service worked collaboratively with the primary care network to improve uptake following a reduction in breast screening attendance and continued to monitor participation rates to support positive population health outcomes.
