We looked for evidence staff involved patients in decisions about their care and treatment and provided them with advice and support. Staff regularly reviewed patient’s care and worked with other services to achieve this. At our last assessment, we rated this key question as good. At this assessment, the rating remains the same.

Supporting people to live healthier lives

The provider supported patients to manage their health and wellbeing to maximise their independence, choice and control. The provider supported patients to live healthier lives and where possible, reduce their future needs for care and support.

Staff focussed on identifying risks to patient’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity.

Monitoring and improving outcomes

The provider routinely monitored patient’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of patients themselves.

The provider met national targets for the uptake of childhood immunisations. The provider performed better than the England average in relation to several cancer screening programmes, including screening for breast, bowel and cervical cancer.

We saw evidence of an established culture of audit activity to monitor the quality of care offered to patients. There had been a wide range of both non-clinical and clinical audits undertaken.
