Supporting people to live healthier lives
The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.
The service supported patients to improve their health and wellbeing through a strong focus on preventative care. Targeted approaches were used to increase cervical screening uptake, particularly for patients less likely to attend. These included structured recall systems, text reminders, online booking, follow-up calls, and flexible clinic times.
Staff also used routine interactions to promote screening and raise awareness. For example, postnatal checks were used to encourage attendance and support patient understanding.
Health promotion was part of everyday practice. Staff provided advice on lifestyle factors such as smoking cessation and diet, and signposted patients to appropriate services including specialist clinics and community programmes. Additional initiatives, such as vaccination clinics and weight management referrals, supported patients to make informed health choices.
The service embedded preventative care into routine practice, ensuring opportunities for early intervention were consistently identified and acted on. This supported improved patient engagement, better health outcomes, and a more proactive approach to long-term wellbeing.
Monitoring and improving outcomes
The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.
The service demonstrated a structured approach to monitoring and improving patient outcomes. A comprehensive audit programme was in place, guided by an annual schedule. Findings were acted on promptly, with improvements re-reviewed to ensure they were effective.
Systems supported safe management of clinical activity, including tracking of urgent referrals and structured handling of clinical correspondence. Tasks were prioritised appropriately, and electronic systems ensured oversight and reduced the risk of delays.
Prescribing practices were regularly reviewed to support safe and appropriate use of medicines, with staff following recognised guidance.
Patients were actively recalled for reviews, including long-term condition monitoring and screening. Structured recall systems supported engagement and improved attendance. Care was well coordinated, with named leads responsible for key clinical areas.
Where possible, appointments were arranged to combine care needs, reducing the need for multiple visits and improving the patient experience. Accurate record-keeping supported continuity of care.
