Supporting people to live healthier lives

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.
The service supported people to manage their health and wellbeing to maximise their independence, choice and control. Health and lifestyle information were displayed in waiting areas, and staff could refer people to a social prescriber through the primary care network (PCN). A PCN is a group of GP practices working together with other health and care providers to deliver coordinated care for local populations.
The service supported people to live healthier lives and, where possible, reduce their future needs for care and support. Staff identified risks to people’s health and wellbeing through working with multidisciplinary teams, for example, wider health services, including mental health services.
Staff focused on identifying risks to people’s health, including for people in the last 12 months of their lives and those with caring responsibilities. Staff attended frailty meetings with community teams to improve outcomes for people. The service took part in pilot initiatives to improve engagement and completion rates for people who had not completed their QFIT test (a test used to detect small amounts of blood in a stool sample, which may indicate the need for further investigation). The service supported national priorities and initiatives to improve population health, including smoking cessation and tackling obesity.
 

Monitoring and improving outcomes

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.
The service had processes to ensure people received appropriate medicines monitoring. This was supported by pharmacists within the PCN, who met monthly with the service and provided regular updates to maintain oversight.
The service carried out clinical audits to support quality improvement, including audits of steroid use on bone health and inhaler use to improve outcomes for people.
The service performed better than the national average for all childhood immunisations. Verified data for cervical screening available at the time of this assessment was from June 2024. This showed the service had met the national uptake target of 80% for people aged 50 to 64 but was slightly below the national uptake target of 80% for people aged 25 to 49. The service provided unverified data, which showed they met targets for cervical screening for both age groups. The service had a process to follow up people who had not booked screening They also provided enhanced access appointments as part of their PCN, this included pre-bookable appointments until 8pm and on Saturdays until 5pm.
