We looked for evidence that staff involved people in decisions about their care and treatment and provided them advice and support. At this inspection we assessed 2 quality statements. The scores have been combined with scores based on the rating from the last inspection. At our last inspection, we rated this key question as Good. At this inspection, the rating remains the same.

Assessing needs

The service made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.

We carried out a series of standardised searches to assess how effectively the practice was assessing and managing the needs of people with selected long-term conditions. We did not identify any concerns.

The clinical team included GPs with special interests in women’s health, neurology, dermatology and frailty. The practice also made use of clinical team members in associated roles funded by the primary care network. The leaders told us that having this range of skills in-house was useful in the context of a population with complex health needs.

People who participated in the assessment were confident that the clinical team understood their individual needs. Staff used digital flags within the care records system to highlight any specific individual needs, such as the requirement for longer appointments or for a translator to be present. Staff checked people’s health, care, and wellbeing needs during health reviews. Clinical staff used templates when conducting care reviews to support the review of people’s wider health and wellbeing. The provider had effective systems to identify people with previously undiagnosed conditions.

Staff could refer people with social needs, such as those experiencing social isolation or housing difficulties, to a social prescriber.

Monitoring and improving outcomes

Score: 2

The service monitored people’s care and treatment to continuously improve. They aimed to ensure that outcomes were positive and consistent. However, there were some areas of performance that were below average, such as cervical screening and childhood immunisations uptake rates that remained below target.

The practice actively followed-up families who had not attended for immunisation in line with the UK schedule but child immunisation uptake rates had not improved over the previous year.
