Governance, management and sustainability

The service had strengthened its governance, management and oversight arrangements. Clear responsibilities, defined roles and systems of accountability were in place and were being used effectively to manage and deliver good quality, sustainable care, treatment and support. Leaders now acted on information relating to risk, performance and outcomes, and shared this securely with relevant stakeholders when appropriate.

Leaders provided structured support to staff, and all staff we spoke with were clear about their individual roles and responsibilities. Regular appraisals and performance reviews were in place and were used to support professional development and identify learning needs. Staff told us these processes were well organised and helped them understand expectations and priorities.

Governance processes were appropriate for the size and scope of the service. Staff had access to relevant policies and procedures, which were kept up to date and reflected current guidance. Leaders had improved internal communication, ensuring staff had timely access to information and opportunities to contribute to service development and improvement.

Leaders had embedded the re-established Patient Participation Group into their governance arrangements, using it to review patient experience data and GP Patient Survey results and to monitor whether improvement actions were having the intended impact over time.

Regular service meetings were now held to discuss clinical concerns, emerging risks and operational issues. These meetings were consistently documented, with actions clearly recorded and shared with the wider team. Staff told us this structured approach had improved oversight and supported a more proactive response to risks.

Leaders had also strengthened oversight of high-risk areas, including medicines governance. Improved monitoring arrangements, audit processes and clinical oversight were now in place to support safe prescribing and ongoing compliance with best practice.
