We looked for evidence staff involved people in decisions about their care and treatment and provided them with advice and support. Supporting people to live healthier lives

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives, and where possible, reduce their future needs for care and support.

Staff focussed on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff at the practice supported national priorities and initiatives to improve population health, and were able to refer patients for services such as smoking cessation, weight management and talking therapies. Working with their primary care network patients also had access to additional support which included social prescribing.

The service offered a range of health checks, assessments, and reviews to help identify need and improve health. These included NHS health checks, new patient assessments and medication checks, annual reviews for patients with a learning disability, and health checks for patients with serious mental health issues. Patient feedback for this latter service was very positive.

Monitoring and improving outcomes

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

Effective invitation and recall procedures were in place, and staff worked to support patients to receive necessary immunisations, vaccinations and screening. The service met national targets for the uptake of childhood immunisations. The service performed better than the England average in relation to cancer screening programmes, including screening for breast, bowel and cervical cancer. As an example, cervical cancer screening rates for both the 25-49 year and the 50-64 year age groups were above the 80% national target at 83.4% and 85.3% respectively. Similarly, performance was higher than average for the treatment of cancers from an urgent suspected cancer referrals with the practice attaining a treatment to referral rate of 69.4% compared to an expected rate of 55.7%. Data provided to us by the practice showed that in the last 12 months they had completed 94% of their annual reviews for patients with a learning disability. The service worked closely with others to improve patient outcomes. This included holding multi-disciplinary team reviews for patients with complex needs and working with the voluntary sector to promote well person reviews in the community.
