Shared direction and culture

Score: 3

The service had a shared vision, values and culture. This was based on respect, teamwork, integrity and learning and improving. Staff and leaders understood the challenges and the needs of people and their communities.

The Partners, Practice Manager and Assistant Practice Manager met weekly to plan and review business arrangements. The service had a business improvement plan which was regularly reviewed. Service leaders demonstrated awareness of local challenges affecting delivery, including the projected growth in the local population over the coming years. They worked collaboratively with partner agencies to address these pressures. The service had strengthened capacity through a variety of workforce initiatives, including supporting and developing a range of clinical roles to ensure that work was undertaken by the most suitably skilled professional.

Leaders actively promoted equality and diversity. Staff we spoke with described the culture as positive and forward thinking which focused on people who used the service and staff. Feedback from both care home and Patient Participation Group representatives was positive about the leadership and supportive culture of the service.

Governance, management and sustainability

Score: 3

The service had clear responsibilities, roles, systems of accountability and good governance. They used these to manage and deliver good quality, sustainable care, treatment and support. They acted on the best information about risk, performance and outcomes, and shared this securely with others when appropriate.

The provider had established governance processes in relation to the quality statements we reviewed during this assessment, which were appropriate for their service. Although we identified the process for the security of prescription stationery was not effective, the provider raised a learning event and took immediate action to identify, improve and implement an effective system.

Leaders held regular departmental meetings with staff, during which they discussed emerging risks, and potential opportunities for service improvement were identified. Team leaders met with the practice manager where risks and ideas were shared. Meeting notes and actions were documented, shared and stored on the service’s computer systems for reference. Staff we spoke with told us these were easily available to them. Staff could access all required policies and procedures, which were kept up to date and reflected latest guidance. All staff we spoke with were clear on their individual roles and responsibilities and those of others. Leaders proactively supported staff and regularly met with them to complete appraisals and performance reviews.

Staff took confidentiality and information security seriously. Staff submitted data and notifications to external agencies as required. The provider was in the process of updating their partnership members and location name.
