During an inspection looking at part of the service

We carried out this announced focused inspection on 14 June 2022 under section 60 of the Health and Social Care Act 2008 as part of our regulatory functions. We planned the inspection to check whether the registered provider was meeting the legal requirements in the Health and Social Care Act 2008 and associated regulations. The inspection was led by a Care Quality Commission (CQC) inspector who was supported by a specialist dental adviser.

To get to the heart of patients’ experiences of care and treatment, we usually ask five key questions, however due to the ongoing pandemic and to reduce time spent on site, only the following three questions were asked:

• Is it safe?

• Is it effective?

• Is it well-led?

These questions form the framework for the areas we look at during the inspection.

Our findings were:

  • The dental clinic was visibly clean and well-maintained.
  • The practice had infection control procedures which reflected published guidance.
  • Staff knew how to deal with medical emergencies. Appropriate medicines and life-saving equipment were available.
  • The practice had systems to help them manage risk to patients and staff.
  • Safeguarding processes were in place and staff knew their responsibilities for safeguarding vulnerable adults and children.
  • The practice had staff recruitment procedures which reflected current legislation.
  • The clinical staff provided patients’ care and treatment in line with current guidelines.
  • Patients were treated with dignity and respect and staff took care to protect their privacy and personal information.
  • Staff provided preventive care and supported patients to ensure better oral health.
  • There was effective leadership and a culture of continuous improvement.
  • Staff felt involved and supported and worked as a team.
  • Staff and patients were asked for feedback about the services provided.
  • The dental clinic had information governance arrangements.

Background

Essex Road DentalShine Practice is in the London Borough of Islington and provides NHS and private dental care and treatment for adults and children.

The practice is located on the ground and lower ground floor of a building close to Essex Road over ground train station. There is level access into the building for people who use wheelchairs or those with pushchairs. The practice has three treatment rooms and a separate decontamination room.

The team includes four dentists, two hygienists, three dental nurses, three receptionists and a practice manager.

During the inspection we spoke with two dentists, the practice manager and a dental nurse. We looked at practice policies and procedures and other records about how the service is managed.

The practice is open:

Monday to Thursday from 9am to 5.30pm.

Friday from 9am to 5pm.

During an inspection looking at part of the service

This visit was a follow up to our inspection of the practice on 13 January 2014.

At that inspection we had found there were ineffective systems in place to reduce the risk and spread of infection. We found that some dental instruments had been wrapped in pouches, however they were not all dated to indicate when they should be reprocessed if left unused. We also saw that some patients were not provided with a disposable protective bib or eye protection whilst receiving treatment at the practice. The provider was not able to produce a cleaning schedule for the practice and there was an inadequate supply of cleaning materials and equipment.

We also had concerns that the practice did not have an adequate system in place to effectively monitor the quality of their service provision.

Following our inspection in January, the practice sent us a plan of the actions intended to meet the requirements of Regulations 10 and 12 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2010. We carried out this visit to check that the actions in that plan had been implemented.

We found that the provider had taken appropriate action and was now compliant with the regulations.

13 January 2014

During a routine inspection

We spoke with seven patients who all gave positive feedback about the care and treatment they had received. One patient told us 'I am very fussy but I was very happy with everything.'

Patients told us that they were given a copy of the treatment and costs involved and given time to think before undergoing treatment. We saw that the practice employed a dentist who specialised in implants and a dental hygienist. This demonstrated that patients had access to appropriate skilled staff to meet their care and treatment needs.

We observed staff speaking to patients politely both at the practice and over the telephone and found they were happy to answer questions. We observed that records relating to patients were kept securely ensuring confidentiality.

The prevention and control of infection and decontamination of instruments was mostly but not always carried out in accordance with guidance from the Department of Health. Patients we spoke with were very positive about the cleanliness of the practice telling us "The surgery is always spotless when I attend."

We found that staff were supported in their professional development and encouraged to take part in additional training.

We had some concerns that the provider did not have an adequate system in place to effectively monitor the quality of their service provision.
