We looked for evidence that staff involved people in decisions about their care and treatment and provided them advice and support. Staff regularly reviewed people’s care and worked with other services to achieve this. At our last assessment, we rated this key question as good. At this assessment, the rating remains the same.

Assessing needs

The service made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. Feedback from people using the service was mixed. Recent survey results from the National GP Patient Survey for the period January to March 2024 showed patient experience indicators for how good the healthcare professional was at listening, confidence and trust in the healthcare professional spoken with and the patients overall experience were below expected. Results provided by the practice from the NHS Friends and Family Test for the period October 2024 to January 2025 showed patients were positive about their experience. Reception staff were aware of the needs of the local community. Care records highlighted any specific individual needs for example, a translator to be present. Staff checked people’s health, care, and wellbeing needs during health reviews. Clinical staff used templates when conducting care reviews to support the review of people’s wider health and wellbeing. Staff could refer people with social needs, such as those experiencing social isolation or housing difficulties, to a social prescriber.

How staff, teams and services work together

Score: 2

The service did not always work well across teams and services to support people. They did not always share their assessment of people’s needs when people moved between different services. Staff had access to the information they needed to appropriately assess, plan, and deliver people’s care, treatment, and support. Practice meetings had recently recommenced under the new practice manager. CQC had received concerns about difficulties in communication between the practice and other health professionals relating to care of patients due to GP requests for information in writing in all cases before responding. The GP and staff told us urgent calls would be put through to the GP or relayed to the GP via a task on the IT system if the GP was attending to a patient appointment. All communication relating to patients was also requested in writing to minimise the risk of miscommunication and to ensure an audit trail for the patients notes. Meetings with community healthcare services to review and ensure seamless care were not undertaken, for example, for complex patients or those at higher risk of hospital admission, and the new practice manager was looking to implement these.
