During an assessment under our new approach

Date of Assessment: 23 January 2025 to 29 January 2025. We assessed this practice following concerns raised with CQC. Barnsley Road Surgery delivers services to 2458 patients under a contract held with NHS England. The National General Practice Profile shows 76% of the patient population is white, 9% Asian, 7% black, 4% mixed and 3% other. Information published by the Office for Health Improvement and Disparities shows that deprivation within the practice population group is in the lowest decile (1 of 10). The lower the decile, the more deprived the practice population is relative to others. This assessment considered the demographics of the people using the service and how this impacted service delivery. We found the service had a limited learning culture and incidents had not always been reported. The facilities and equipment met the needs of people, but not all risks were mitigated. There were enough staff, but recruitment checks had not always been completed. There was a lack of evidence staff had the appropriate qualifications and experience. Staff were not always up to date with training and clinical supervision was not provided. Medicines were not appropriately managed and stored safely. Emergency equipment was not regularly checked. Care was based on latest evidence and good practice. People knew how to give feedback, and the service took it seriously and acted on it. The service was accessible although patients gave mixed feedback about contacting the practice. Leaders were visible, knowledgeable and supportive. Staff understood their roles and responsibilities but had limited time to complete tasks and had not always followed procedures. There was limited oversight from the provider of staff, implementation of policies and management of risk. We found breaches of regulation in relation to safe care and treatment and employment of fit and proper persons. We have asked the provider for an action plan in response to the concerns found at this assessment.

We carried out a targeted assessment of the responsive key question. Targeted assessments enable us to focus on certain key questions to explore particular aspects of care.

How we carried out the assessment

  • This assessment was carried out without a site visit.
  • Conducting staff interviews using video conferencing.
  • Requesting evidence from the provider and reviewing the appointment system.

Our findings

We based our judgement of the quality of care at this service on a combination of:

  • what we found when we carried out the assessment
  • information from our ongoing monitoring of data about services and
  • information from the provider, patients, the public and other organisations.

We found that:

  • Patients could access care and treatment in a timely way and the provider had implemented systems and processes as a result of patient feedback.
  • National GP patient survey results relating to access were above national averages.
  • Complaints were satisfactorily handled in a timely manner.

During a routine inspection

We carried out an announced comprehensive inspection at Barnsley Road Surgery on 21 May 2019 as a new provider had registered with CQC to provide regulated activities from this location in July 2018.

We based our judgement of the quality of care at this service on a combination of:

• what we found when we inspected

• information from our ongoing monitoring of data about services and

• information from the provider, patients, the public and other organisations.

This practice is rated as Good overall.

At this inspection we found:

  • The practice provided care in a way that kept patients safe and protected them from avoidable harm.
  • Patients received effective care and treatment that met their needs.
  • Staff dealt with patients with kindness and respect and involved them in decisions about their care.
  • The practice organised and delivered services to meet patients’ needs. Patients could access care and treatment in a timely way.
  • The way the practice was led and managed promoted the delivery of high-quality, person-centre care.
