Favell Plus Surgery is a GP practice and delivers services to residents in Weston Favell and the surrounding areas under a contract held with NHS England. Information published by Office for Health Improvement and Disparities shows that deprivation within the practice population group is in the 3rd decile (3 of 10). The lower the decile, the more deprived the practice population is relative to others. At the time of our assessment there were 12,682 patients registered with the practice (77% White, 8% Asian, 10% Black and 5% from Mixed or Other ethnicities).
We carried out an announced comprehensive assessment of all 5 key questions (Safe, Effective, Caring, Responsive and Well-Led) between 4 November 2025 and 11 November 2025. This assessment was carried out to provide a rating and assess the quality of services being delivered as we received some information of concern that we wanted to follow up on.
During the assessment, we found the service was providing safe, effective, caring, and responsive services. However, we found concerns related to effective management oversight, governance systems and clinical coding of patients’ records which resulted in breach of Regulation 17 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. We have asked the provider for an action plan in response to the concerns found at this assessment.
