Letter from the Chief Inspector of General Practice

We carried out an announced comprehensive inspection at Dr Vipan Palta (also known as Auriol Medical Centre) on 28 April 2016. During this inspection we found a breach of legal requirements and the provider was rated as requires improvement under the safe domain. The practice sent to us an action plan detailing what they would do to meet the legal requirements in relation to the following:-

  • Ensure all staff undertake mandatory training including Safeguarding Vulnerable Adults and Safeguarding Children, fire awareness, infection control and information governance.
  • Ensure electrical items are PAT tested and medical equipment is calibrated.
  • Complete regular fire drills.

We undertook this announced focused inspection on 20 December 2016 to check the provider had followed their action plan and to confirm that they now met legal requirements. The provider was now meeting all requirements and is rated as good under the safe domain.

Our key findings across all the areas we inspected were as follows

  • Staff had undertaken on-line training for topics the practice had considered as mandatory. This included Safeguarding Vulnerable Adults and Safeguarding Children, fire awareness, infection control and information governance.
  • All electrical items had been PAT tested (portable appliance testing) by an independent company and all medical equipment had been calibrated to ensure they worked correctly.
  • Staff had undertaken fire drills.

We carried out an announced comprehensive inspection at Dr Vipan Palta (also known as Auriol Medical Centre) on 28 April 2016. Overall the practice is rated as good.

Our key findings across all the areas we inspected were as follows:

  • There was an open and transparent approach to safety and an effective system in place for reporting and recording significant events.
  • Risks to patients were assessed and well managed. With the exception of practicing fire drills, testing of electrical equipment and the calibration of medical equipment.
  • Staff assessed patients’ needs and delivered care in line with current evidence based guidance. Staff had been trained to provide them with the skills, knowledge and experience to deliver effective care and treatment. However, non clinical staff had not been provided with adequate training.
  • Patients said they were treated with compassion, dignity and respect and they were involved in their care and decisions about their treatment.
  • Information about services and how to complain was available and easy to understand. Improvements were made to the quality of care as a result of complaints and concerns.
  • Patients said they found it easy to make an appointment with a named GP and there was continuity of care, with urgent appointments available the same day.
  • The practice had good facilities and was well equipped to treat patients and meet their needs.
  • There was a clear leadership structure and staff felt supported by management. The practice sought feedback from staff and patients, which it acted on.
  • The provider was aware of and complied with the requirements of the duty of candour.
  • The practice was able to offer evening appointments (until 9.30pm) and weekend appointments to all their patients. (A hub of doctors’ practices within the clinical commissioning group area jointly ran this service from two separate locations in Leatherhead and Epsom.)

The areas where the provider must make improvement are:

  • Ensure all staff undertake mandatory training including Safeguarding Vulnerable Adults and Safeguarding Children, fire awareness, infection control and information governance.
  • Ensure electrical items are PAT tested and medical equipment is calibrated.
  • Complete regular fire drills.

In addition the provider should:

  • Review how risk assessments are recorded
  • Review how to engage patients to join the patient participant group.

During a routine inspection

We spoke with two patients and four members of staff which included the GP. We also collected five responses to a patient survey as part of our inspection to Auriol Medical Centre.

We noted that staff closed doors of the treatment and consulting rooms which ensured privacy and dignity for patients. The patients that we spoke with and the results of the patient surveys told us that everyone felt respected by the staff at the practice. One patient told us they were 'Outstanding.'

We saw that the practice had safeguarding policies and procedures that related to adults and children. Staff knew what signs to look for that related to abuse.

On the day of the inspection we found that the practice seemed clean and hygienic.

We found that staff within the practice received annual appraisals and were supported in their work.

We saw that the practice had a complaints policy and that this was made available to patients.
