During an inspection looking at part of the service

Our inspection of 10 September 2013 found that patients were not protected because the provider did not operate an effective recruitment process or make risk assessments in respect of staff roles.

During this inspection we saw that a recruitment policy was in place that was followed during the recruitment of new staff. The provider had compiled personnel files for all staff and these included a full work history and evidence of their identity. The provider had also requested checks from the Disclosure and Barring Service (DBS) for all staff.

During a routine inspection

During our inspection we spoke with the practice manager, two doctors, the practice nurse, a receptionist, an apprentice receptionist/medical secretary and a volunteer receptionist. One patient agreed to speak with us. We took into consideration the results of the latest NHS GP Patient Survey.

We saw that patients were given appropriate information and support regarding their care or treatment. There was no equipment available, such as oxygen, to use in the case of a patient emergency.

Most staff had not been trained in safeguarding, and some were unaware there was a safeguarding policy in place. The provider did not have effective systems in place to assess the risk of and to prevent, detect and control the spread of infection. The provider did not operate an effective recruitment process. There were not enough qualified, skilled and experienced staff to safeguard people's health, safety and welfare.

The provider did not have suitable arrangements in place to ensure that staff received appropriate training. There was no system in place for the supervision and appraisal of staff. There was no effective quality assurance system in place. The patient we spoke with gave mainly positive feedback about the practice. They told us they understood explanations about their conditions and treatments.
