During a routine inspection

Letter from the Chief Inspector of General Practice

We carried out an announced comprehensive inspection at Parsonage Surgery on 14 July 2016. Overall the practice is rated as good.

Our key findings across all the areas we inspected were as follows:

  • There was an open and transparent approach to safety and an effective system in place for reporting and recording significant events.
  • Risks to patients were assessed and well managed. However systems relating recording of monitoring information of historic safety alerts and children not attending hospital appointments (DNA) needed strengthening.
  • Staff assessed patients’ needs and delivered care in line with current evidence based guidance. Staff had been trained to provide them with the skills, knowledge and experience to deliver effective care and treatment.
  • Patients said they were treated with compassion, dignity and respect and they were involved in their care and decisions about their treatment.
  • Information about services and how to complain was available and easy to understand. Improvements were made to the quality of care as a result of complaints and concerns.
  • Patients said they found it easy to make an appointment with a named GP and there was continuity of care, with urgent appointments available the same day.
  • The practice had good facilities and was well equipped to treat patients and meet their needs.
  • There was a clear leadership structure and staff felt supported by management. The practice proactively sought feedback from staff and patients, which it acted on.
  • The provider was aware of and complied with the requirements of the duty of candour.

The areas where the provider should:

  • Continue to monitor the recently implemented system to monitor historic safety alerts.

  • Continue to monitor the recently implemented protocol to code those children who had failed to attend a hospital appointment (DNA) so they could be easily identified and acted on.

  • Ensure modifications to the access doors to the practice are carried through as planned in conjunction with the facilities directorate of Essex Community Hospital so access to patients with limited mobility can be improved.

  • Continue to monitor patient survey results and ensure improvement to access to the practice over the phone.

During a routine inspection

We found the service to be welcoming with friendly staff. Practice information was displayed for people who used the service, including health promotion, access to support and other available services. Appointments could be made in person at the surgery, by telephone, or using an on line system via the practice's website.

We spoke with nine people who all spoke highly of the services provided to them. We also spoke with staff who said they enjoyed working in the practice.

People's needs were assessed and care and treatment was planned and delivered in line with their individual wishes. One person said, "Everyone here bends over backwards to help. This surgery is the best one I have ever been to." Another person said, 'Fantastic. I can't fault it.'

We saw that there was a system to ensure repeat prescriptions were available promptly and medicines that were kept at the practice were stored safely.

Staff were supported through appraisal, informal supervision and staff meetings.

All records were fit for purpose and stored securely. Paper records were disposed of safely and appropriately.
