Supporting people to live healthier lives
The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.
Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity. The service provided information in various formats such as discussion and signposting, displays on notice boards and information on the website. The service provided access to a mental health practitioner and offered services such as a social prescriber and healthy eating support
Results from the most recent National Patient Survey (2025) were that 92% of people felt their needs were met during their last general practice appointment; this is above the national (90%) and local (88%) averages.
Monitoring and improving outcomes

Data from the UK Health Security Agency states that the service had not achieved any of the 5 targets set for children immunisation for the 12-month period up to end March 2025, for example children aged 1 who had completed the recommended course of immunisation was 87.9%, the expected minimal target is 95%.
Verified data from NHS England, June 2024, showed 74.7% of eligible women in the age bracket 25 to 49 years old had an adequate screening test, the expected minimal target is 80%. We did view unverified data, shared by the service, which showed an improvement on these figures.
The service had a recall system. Leaders and staff were able to demonstrate an awareness of the local population and the challenges that affected vaccination uptake. They provided examples of discussions with patients to ensure people had the information they needed to make informed decisions about their care and treatment. Staff also demonstrated times when there was a flexible and person-centred approach. For example, when a vaccine was declined due to concerns about ingredients, staff worked to identify and offer a suitable alternative where available.
The service were actively encouraging people to take part in bowel cancer screening. Staff described clear and informative discussions with people who used the service to ensure they were aware of the importance of screening tests and the positive outcome they can contribute to. Unverified data from Department of Health and Social Care indicated that the service had seen an increase in the number of screening tests for bowel cancer.
