During a routine inspection

We carried out this announced comprehensive inspection on 9 January 2023 under section 60 of the Health and Social Care Act 2008 as part of our regulatory functions.

We planned the inspection to check whether the registered practice was meeting the legal requirements in the Health and Social Care Act 2008 and associated regulations.

The inspection was led by a Care Quality Commission (CQC) inspector who was supported by a specialist dental advisor.

To get to the heart of patients’ experiences of care and treatment, we always ask the following 5 questions:

  • Is it safe?
  • Is it effective?
  • Is it caring?
  • Is it responsive to people’s needs?
  • Is it well-led?

These questions form the framework for the areas we look at during the inspection.

Our findings were:

  • The dental clinic appeared clean and well-maintained.
  • The practice had infection control procedures which reflected published guidance.
  • Staff knew how to deal with medical emergencies. Appropriate medicines and life-saving equipment were available.
  • The practice had systems to manage risks for patients, staff, equipment and the premises.
  • Safeguarding processes were in place and staff knew their responsibilities for safeguarding vulnerable adults and children.
  • The practice had staff recruitment procedures which reflected current legislation.
  • Clinical staff provided patients’ care and treatment in line with current guidelines.
  • Patients were treated with dignity and respect. Staff took care to protect patients’ privacy and personal information.
  • Staff provided preventive care and supported patients to ensure better oral health.
  • The appointment system worked efficiently to respond to patients’ needs.
  • The frequency of appointments was agreed between the dentist and the patient, giving due regard to National Institute of Health and Care Excellence (NICE) guidelines.
  • There was effective leadership and a culture of continuous improvement.
  • Staff felt involved, supported and worked as a team.
  • Staff and patients were asked for feedback about the services provided.
  • Complaints were dealt with positively and efficiently.
  • The practice had information governance arrangements.

Background

Thompson and Thomas LLP is in Sheffield and provides NHS and private dental care and treatment for adults and children.

There is a small step into the practice, patients who require step free access are directed to the sister practice a short distance away. Car parking spaces are available near the practice on local side roads.

The practice has made reasonable adjustments to support patients with access requirements; staff will assist patients into the practice if this is requested.

The dental team includes 6 dentists (1 of whom is a foundation dentist), 7 dental nurses (2 of whom are trainees) and a practice manager. Dental nurses also cover reception duties on rotation. The practice has 4 treatment rooms.

During the inspection we spoke with 3 dentists, 3 dental nurses and the practice manager. We looked at practice policies, procedures and other records to assess how the service is managed.

The practice is open: Monday to Wednesday 8:30 – 5:30pm, Thursday 8:30 – 5pm and Friday 8:30am – 4pm.

During a routine inspection

People told us that the dentist explained all the procedures and treatments to them and respected their decisions about their treatment and care. Their comments included; 'the dentist is very good. He always explains everything and I can decide,' and 'the dentist always tells me what needs to be done, and explains any choices to me. They listen here.'

We spoke privately with three people who were attending the practice for check ups or treatment. All three people told us that they were happy with all aspects of the service. People commented; "it really is excellent here. The staff are friendly. The dentist always does what is best for me," and "I cannot fault the service I get. All my family come here and we are all really happy."

People spoken with said that they always found the practice clean, tidy and hygienic. We found people received care and treatment in a clean environment with infection control measures in place to minimise the risk of infection.

The provider had a satisfactory recruitment and selection procedure in place to ensure that staff were appropriately employed. Staff were provided with relevant training to maintain and update their skills and knowledge.

The provider had an appropriate system in place for gathering, recording and evaluating information about the quality and safety of care the service provides. People who used the service were asked for their views about their care and treatment.
