During a routine inspection
The sonographer was able to describe procedures for managing emergencies, and had undergone relevant training. There was no emergency equipment or medicines available, and no documented emergency procedures. There were no arrangements in place to assess or monitor the continued safety and suitability of the premises.
Manufacturer's guidelines were available to guide the use of ultrasound equipment, but there was no recorded evidence that the sonographer had undergone specific training to use the equipment, or that equipment used as part of the regulated activity was properly maintained, serviced or renewed under a recorded programme.
The sonographer we spoke with had completed some training : however it was not specific to current UK ultrasound practices.
The provider did not have a formal system to regularly assess and monitor risk or the quality of the service that people received and could not provide us with any examples of applying learning from local or national service reviews, audits or research projects.
