About the service

Keegan’s Court Residential Care Home is a care home providing support with personal care to 2 people at the time of this inspection. The home can accommodate a maximum of 19 older people. Accommodation is provided in an adapted building providing 15 beds and 2 bungalows, each providing 2 beds.

People's experience of using this service and what we found

This was a targeted inspection that considered whether or not people were safely supported by enough staff who had sufficient training, information and managerial direction to minimise the potential for harm.

People were at the risk of harm as the provider had failed to provide sufficient staff to safely support them at certain times of the day and night. Staff were not provided with clear instructions on what to do in the event of an emergency.

People were not safeguarded from the risk of abuse as the provider failed to assess the risks to people from a known individual entering the building.

The provider did not have effective oversite to ensure people received safe care.

Rating at last inspection and update.

The last rating for this service was inadequate (published 30 March 2023). At our last inspection the provider was in breach of regulations regarding keeping people safe, dignity and respect, safeguarding people from abuse, staffing and the overall management. At this inspection concerns remained regarding the safe provision of care, safeguarding people from abuse and managerial oversite.

Why we inspected

The inspection was prompted due to concerns received about unsafe staffing levels in the home. A decision was made for us to inspect and examine those risks. We found continued breaches regarding keeping people safe, safeguarding people from abuse and governance of the home. Following the inspection site visit the provider told us they had employed the services of a managerial consultancy to support them with restructuring their company.

The overall rating for the service has not changed following this targeted inspection and remains inadequate.

CQC have introduced targeted inspections to follow up on a Warning Notice or other specific concerns. About the service

Keegan’s Court Residential Care Home is a care home providing support with personal care to 3 people at the time of this inspection. The home can accommodate a maximum of 19 older people. Accommodation is provided in an adapted building providing 15 beds in the main building and two bungalows, each providing 2 beds.

People’s experience of using this service and what we found

People were not safe from avoidable harm. Despite being aware of the lack of heating fuel, the provider had failed to purchase supplies resulting in the heating failing. This resulted in the physical temperature of the home reducing to an uncomfortable level increasing the risk of harm to people.

People were not protected from the risk of abuse or ill-treatment. The provider failed to act when concerns regarding staff members conduct were identified and passed to them. This put people at the risk of receiving inappropriate or potentially abusive behaviour.

People were not treated with dignity or respect. Confidential information was not secured and was accessible to those without authority. This information contained sensitive financial information and information regarding people’s health and welfare. People’s privacy could not be assured as communal toilet doors did not close or could not be locked.

The provider failed to ensure an effective fire management plan was in place, known, and followed by staff supporting people. This put people at the risk of harm in the event of an emergency. The provider failed to assess potential risks to people putting them at the risk of harm. The provider failed to ensure there were enough suitably qualified staff available to support people at all times.

People did not always receive their medicines safely or as prescribed. The provider failed to safely store medicines and failed to ensure medicines were administered in line with the guidance provided. The provider did not effectively analyse significant incidents to learn from them and to make changes to improve people’s safety. People were not effectively protected from the risk of infection as the provider did not have adequate infection prevention and control measures in place. There was visible dirt in areas of the home and on fixtures. The provider had failed to update or implement the latest infection prevention and control guidance. The provider failed to complete checks on visitors to ensure they did not pose a risk to people.

The provider did not have effective quality monitoring procedures in place to drive improvements in the care they provided. The management team did not have clearly defined roles and responsibilities. The provider failed to have effective contingency plans in place regarding the restructuring of their business.

The provider followed safe recruitment practices.

People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests; the policies and systems in the service supported this practice.

About the service

Keegan’s Court Residential Care Home is a care home providing support with personal care to 15 people at the time of this inspection, some of whom were living with dementia. The home can accommodate a maximum of 19 older people. Accommodation is provided in an adapted building providing 15 beds in the main building and two bungalows, each providing two beds.

People’s experience of using this service and what we found

People were not always treated with dignity or respect. Confidential information was not secured and was accessible to those without authority.

People were not safe as the provider failed to ensure the physical environment was safely maintained and systems and processes were not effectively followed. Including, but not limited to, ineffective fire safety systems, unsafe storage of chemicals and lack of identification of risks with associated with windows and the pond area.

People were not always protected from the risks of abuse. People did not always receive their medicines safely or as prescribed.

The provider did not effectively analyse significant incidents to learn from them and to make changes to improve people’s safety. The providers infection prevention and control procedures were not effectively followed.

The provider did not have effective quality monitoring procedures in place to drive improvements in the care they provided. The management team did not have clearly defined roles and responsibilities.

People were not always supported to have maximum choice and control of their lives. Staff did not always support them in the least restrictive way possible or in their best interests; the application of policies and systems in the service did not always support best practice.

The provider followed safe recruitment practices.

The last rating for this service was requires improvement (published 25 February 2022).

At that inspection there were breaches of regulation regarding safe care and treatment and how the location was managed. Following that inspection, the provider was issued with warning notices.

We undertook a targeted inspection on 12 April 2022 to check they had complied with the warning notice. We use targeted inspections to follow up on Warning Notices or to check concerns. They do not look at an entire key question, only the part of the key question we are specifically concerned about. Targeted inspections do not change the rating from the previous inspection. This is because they do not assess all areas of a key question. Following the targeted inspection, we confirmed the provider was meeting the legal requirements and the conditions of the warning notice.

We are mindful of the impact of the COVID-19 pandemic on our regulatory function. This meant we took account of the exceptional circumstances arising as a result of the COVID-19 pandemic when considering what enforcement action was necessary and proportionate to keep people safe as a result of this inspection. We will continue to monitor the service and will take further action if needed.

We have identified breaches in relation to keeping people safe, dignity and overall governance.

The home can accommodate a maximum of 19 older people. Accommodation is provided in an adapted building providing 15 beds and two bungalows in the grounds.

People's experience of using this service and what we found

People were safe as the infection prevention and control procedures had been revised by the provider and effectively implemented.

The provider had undertaken work to correct potential hazards to people.

The provider had introduced quality monitoring systems in order to drive good care.

At this inspection we found improvements had been made and the provider was no longer in breach of regulations.

The provider did not have effective systems in place to identify environmental issues which could put people at the risk of harm. The provider did not have effective quality monitoring procedures to drive good care.

People received their medicines as prescribed. Staff understood how to protect people from the risk of abuse and knew what to do if they suspected something was wrong. The provider followed safe recruitment practices.

The provider had kept us informed about key events and had good working relationships with others involved in peoples care.

We looked at infection prevention and control measures under the Safe key question. We look at this in all care home inspections even if no concerns or risks have been identified. This is to provide assurance that the service can respond to COVID-19 and other infection outbreaks effectively. This included checking the provider was meeting COVID-19 vaccination requirements.

Following our inspection site visit the provider took action to mitigate the immediate risks to people including conformation of the vaccination status of staff members.

About the service

Keegan’s Court Residential Care Home is a care home providing support with personal care needs to seven people at the time of this inspection. The home can accommodate a maximum of 19 older people. Accommodation is provided in an adapted building providing 15 beds and two bungalows in the grounds, each providing two beds.

People’s experience of using this service and what we found

Systems to assess, monitor and mitigate risks to people’s safety and well-being had improved. Further time is needed to ensure improvements are embedded and can be sustained over time. People were protected by the provider’s staff recruitment procedures. Staff received the support and training they needed to meet people’s needs. People received their medicines when they needed them from staff who were trained and competent to carry out the task. Infection, prevention, control procedures had improved and helped to protect people from the risk of infection.

People received a service which took into account their needs and preferences. However, further improvements were needed to ensure people’s religious preferences and wishes for care at the end of their life and following death were understood and met by staff.

Improvements had been made to ensure the service was effectively managed. Systems had been introduced to monitor the quality and safety of the service provided. The provider worked in partnership with others to achieve good outcomes for people. These included health and social care professionals and stakeholders.

This service has been in Special Measures since February 2021. During this inspection the provider demonstrated that improvements have been made. The service is no longer rated as inadequate overall or in any of the key questions. Therefore, this service is no longer in Special Measures.

We undertook this focused inspection to check they had followed their action plan and to confirm they now met legal requirements. This report only covers our findings in relation to the Key Questions; safe, responsive and well-led which contain those requirements.

About the service

Keegan’s Court Residential Care Home is a care home providing support with personal care needs to a maximum of 19 older people. Accommodation is provided in an adapted building providing 15 beds and two bungalows in the grounds, each providing two beds. At the time of the inspection, 14 people were using the service.

People’s experience of using this service and what we found

Risks to people’s safety and well-being were not always considered and plans to mitigate risks were not in place or had not been reviewed.

Action had not been taken to reduce the risk of scalding from hot water outlets. Information about how people should be safely evacuated in the event of an emergency was not available for each person who lived at the home.

People were not protected by the procedures for the management and administration of medicines.

Infection, prevention and control procedures did not protect people from the risk of infection.

People’s experience of using this service and what we found

Risks to people’s safety and well-being were not always considered and plans to mitigate risks were not in place or had not been reviewed.

People were not protected by the provider’s staff recruitment procedures. The provider failed to ensure staff received the required support to meet people’s needs safely.

Infection, prevention, control procedures did not protect people from the risk of infection.

People were at risk of not receiving a service which met their needs and preferences.

The service was not effectively managed and there were no systems in place to monitor the quality and safety of the service provided. The provider had failed to act on the breaches of regulations identified at our last inspection.

During a routine inspection

About the service: Keegan’s Court is a care home. People in care homes receive accommodation and nursing or personal care as single package under one contractual agreement. CQC regulates both the premises and the care provided, and both were looked at during this inspection. Keegan’s Court accommodates up to 19 older people who require support with their personal care needs. At the time of this inspection, 16 people were living at the home.

People’s experience of using this service:

• People told us they felt safe living at the home and with the staff that supported them. However, risks to people were not always monitored or mitigated. People received their medicines when they needed them however, the arrangements for recording medicines could place people at risk. Free standing wardrobes had not been secured which meant they posed a potential hazard to people.

• People were supported by adequate numbers of staff who were safe and competent to work with them.

• People were protected from the risks associated with the control and spread of infection.

• Staff understood the importance of ensuring people’s rights were understood and protected.

• People’s health care and nutritional needs were monitored and understood by staff.

• People told us staff understood their needs and were kind, caring and compassionate.

• People had opportunities for social stimulation and were able to maintain links with the local community.

Rating at last inspection: This was the first inspection of the service since it registered with the Commission in November 2017.

Why we inspected: This was a scheduled inspection to provide the service with a rating.

During a routine inspection

About the service

Keegan’s Court Residential Care Home accommodates up to 19 older people who require support with their personal care needs. Some of whom are living with dementia. At the time of this 14 people were living there.

People's experience of using this service and what we found

The providers quality monitoring procedures were effective in identifying the improvements which were needed to drive good care and support.

People were not always safe as the systems and procedures at Keegan’s Court Residential Care Home were ineffective in identifying improvements needed to keep people safe. Risks associated with people’s care were not always accurately identified. People were not safe from infectious illnesses as the infection prevention and control measures at Keegan’s Court Residential Care Home were not always effective. People's 'As required' medicine protocols were incomplete and in some instances missing.

People were not always supported to have maximum choice and control of their lives and they were not always supported in the least restrictive way possible or in their best interests; the policies and systems in the service did not always support good practice. People were not always treated with dignity and respect and information personal and confidential, to them, was not always kept secure.

People did not always have personalised care and support plans which reflected their individual preferences. People were not consistently provided with information in a way they could easily access or understand as their communication styles had not been effectively assessed.

People did not always receive effective support with their eating and drinking as there was no oversight of records detailing people’s intake.

People were protected from harm and abuse as the staff team had been trained to recognise potential signs of abuse and understood what to do.

People had access to additional healthcare services when required.

People received help and support from a kind and compassionate staff team with whom they had developed positive relationships. The provider had systems in place to encourage and respond to any complaints or compliments from people or those close to them.

Rating at last inspection

The last rating for this service was 'Requires Improvement' (published 27 February 2019). At that inspection we identified breaches in the law regarding the notification of incidents, safe care and treatment and quality assurance systems.

Why we inspected

This was a planned inspection based on the previous rating.

The overall rating for the service has remained the same, requires improvement. This is based on the findings at this inspection.

We have found evidence that the provider needs to make improvement in all key questions inspected. Please see the Safe, Effective, Caring, Responsive and Well-led sections of this full report.

Enforcement:

We have identified breaches in relation to the safe care and treatment of people, person-centred care planning and how the location is managed.
