Responsive – this means we looked for evidence that the provider met people’s needs.

At our last assessment we rated this key question requires improvement. At this assessment the rating has changed to good. This meant people’s needs were met through good organisation and delivery.

Care provision, Integration and continuity

Score: 3

The provider understood the diverse health and care needs of people and their local communities, so care was joined-up, flexible and supported choice and continuity.

People had continuity of care and were each supported by a team of regular staff who understood them well. The provider did not use agency staff.

Staff received a range of training, to enable them to understand people’s care needs and how best to support them. Staff supporting people with mental health needs and or a learning disability had received training relevant to the needs of the person they supported.

Staff worked with a range of professionals across different teams and agencies to plan and deliver people’s care. The provider identified any changes in the level of people's support needs which were then shared with commissioners, such as a change in their care needs.

Providing Information

The provider supplied appropriate, accurate and up-to-date information in formats that were tailored to individual needs.

Staff told us about how people preferred information to be provided, verbally, in writing or visually. People were provided with easy read versions of their care plan where required. People had activity schedules where needed to support their understanding of their day and what was happening and when. Staff also used social stories with people who needed this support, to enable them to understand social situations and events in their lives.

Equity in access

The provider made sure that people could access the care, support and treatment they needed when they needed it.

People received care and support when they needed it in a way that worked for them. The management team and staff team understood the needs of people with a learning disability and autistic people and tried to ensure there were no barriers to people receiving the specialist support required

Some people required high levels of staff support 24 hours a day whilst other people were more independent. Each person’s staffing level was planned around their needs.

Staff listened to people and worked with them in order to understand their needs and to support their safety. Staff supported people in accordance with their behaviour support plan.

The provider ensured people’s home environment was suitable for their needs and safe for them. People had the physical space they needed, and any adaptions required for their safety within their home.

Equity in experiences and outcomes

Staff and leaders actively listened to information about people who are most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this.

Staff supported people to be present in their community and to participate in a range of activities planned with them which reflected their personal interests. Relatives told us people were able to access a range of activities, which included hobbies, education, personal interests and the use of various community facilities. A person had recently moved accommodation and staff told us how the person enjoyed meeting people in their new community and being able to do more activities.
