At our last assessment we rated this key question good. At this assessment the rating has remained good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.

Care plans were outcome-focused and regularly reviewed. People were encouraged to set personal goals and were supported to achieve them at their own pace. We saw examples of people having new experiences by visiting different places, attending health appointments and meeting new people and making new relationships.

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards.

A relative said, “My [relative] support needs have changed, HF Trust are responding well to this I feel, they changed around their room, so it is safer and got equipment for them to use.”

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people were able to share what was important to them and only needed to tell their story once by sharing their assessment of needs when people moved between different services.

When speaking with staff they knew people well. Professionals also confirmed that when speaking with staff they were able to share what was important to the person.

Supporting people to live healthier lives

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice, for example, where incidents relating to medicines occurred the management identified these and shared lessons learnt with the wider team.

Monitoring and improving outcomes

People were supported to regularly review the support they received from the provider and other health agencies. They ensured that outcomes were positive, and people were involved in those discussions. For example, a person felt very anxious about visiting the dentist. Information about the visit was shared in a format that was meaningful to the person. With this, and visits to see the environment, the person was able to attend.
