The practice had processes to identify and manage risks, but improvement was needed to ensure processes were effective.

Staff could access emergency equipment and medicines that were checked in line with national guidance. Improvement was needed to ensure the storage arrangements used for Glucagon (an emergency medicine used to regulate blood sugar levels) followed manufacturer’s guidance.

Staff knew how to respond to a medical emergency and had completed training in emergency resuscitation and basic life support every year.

The premises were visibly clean and free from clutter.

Environmental cleaning schedules, specific to the practice, were not available.

Oversight of environmental cleaning standards could not be demonstrated.

Improvement was needed to ensure the safe storage of every substance hazardous to health that was in use by staff.

The practice had arrangements to ensure the safety of the X-ray equipment, but improvement was needed to ensure that servicing of equipment was carried out in line with manufacturer’s instructions.

We do not assess compliance with the Ionising Radiation regulations 2017 and the Ionising Radiation (Medical Exposure) regulations 2017 but we do request services to provide evidence that demonstrates their compliance to inform our findings.

The practice’s oversight of fire safety management procedures were not effective.

The practice had systems for appropriate and safe management of medicines.

NHS prescription pads were kept securely, and a log was in place to monitor and track their use.

Improvement was needed to the practice's systems for checking and monitoring equipment to ensure equipment is available and well maintained. In particular, air-conditioning servicing and the availability of carbon monoxide detectors.

The practice had a recruitment policy and procedures that reflected relevant legislation. However, these were not consistently followed and oversight of recruitment checks required improvement.

The practice ensured clinical staff were qualified, registered with the General Dental Council and had appropriate professional indemnity cover.

Improvements should be made to take action to ensure that all clinical staff have adequate immunity for vaccine preventable infectious diseases.

The practice could not demonstrate that every new member of staff had an appropriate role specific structured induction.

Staff demonstrated knowledge of safeguarding and were aware of how safeguarding information could be accessed. Staff knew how to escalate safeguarding concerns within the practice and externally.

Oversight of staff training, including continuing professional development required improvement.

Annual appraisals were not carried out.

Staff felt respected and valued, and they were proud to work in the practice.

Improvement was needed to ensure that infection control procedures reflected published guidance.

Staff used personal protective equipment and decontaminated dental instruments after use, in line with national guidance. We saw, and staff confirmed that single-use items were not reprocessed.

Oversight of treatment room cleaning standards could not be demonstrated.

The practice had effective procedures to reduce the risk of Legionella, or other bacteria, developing in water systems, in line with a risk assessment and current guidance.

Staff received appropriate training and demonstrated knowledge and awareness of infection prevention and control processes.

The practice had protocols to ensure effective cleaning and safe segregation and disposal of hazardous waste, but improvement was needed to ensure the external clinical waste was secure.

The practice completed infection prevention and control audits in line with current guidance. However, improvement was needed to ensure audits were carried out at the recommended intervals.
