Assessing needs

Score: 3

The provider assessed people’s needs and choices before they moved to the service. They consulted the person, their relatives and professionals involved in their care. A relative explained, “When [person] moved to the home there was lots of planning and preparation. Staff understand [their] needs.” Staff undertook regular reviews and reassessments to make sure planned care met people’s needs.

Delivering evidence-based care and treatment

Staff delivered evidence-based care. The provider arranged for staff to undertake a wide range of training, including training about people with a learning disability and how to manage challenging situations. Staff told us training was useful and helped them with their work. Comments from staff included, “The training has strengthened my understanding of how to provide respectful structured support to meet individual needs” and “Training covers a wide range of topics. There is also informal learning through staff meetings and discussions.”

How staff, teams and services work together

Staff communicated well with each other. The staff team at each supported living setting had systems to update each other and make sure they provided consistent care. The provider also ensured staff teams shared good practice and learning from incidents across the different services.

External professionals told us the staff communicated well with them, providing information and acting on their guidance. An external healthcare worker commented, “Staff have always dealt with me professionally. We maintain healthy and effective communication.” Recommendations from external professionals were incorporated into care plans.

Supporting people to live healthier lives

The provider supported people with their healthcare needs. Staff assessed their needs and worked with healthcare professionals to monitor and meet these. Staff supported people with their weight management and making healthy choices about diet and exercise. Relatives of people told us they were pleased with the support people received to stay healthy and manage their weight.

Monitoring and improving outcomes

Staff monitored people’s wellbeing to make sure they were receiving the right care and support. People were assigned a key member of staff who regularly reviewed their care plans. People discussed their needs with staff and planned their own objectives and goals. These were regularly reviewed to help ensure people were getting the right support to achieve these.

The provider made sure people consented to their care and treatment. People confirmed staff offered them choices and asked for consent. We observed staff offering choices in ways people could understand and respecting their decisions. People were involved in planning their own care. People were asked for their opinions and were asked to consent to different aspects of the service. Some people did not have the mental capacity to make certain decisions. Staff assessed this and worked with people’s representatives to make sure decisions were made in their best interests.
