We found this practice was not providing well-led care in accordance with the relevant regulations.

The provider had made insufficient improvements to put right the lack of systems or processes that enabled the registered person to assess, monitor and improve the quality and safety of the services being provided. There was also insufficient improvement to put right the lack of systems or processes to assess, monitor and mitigate the risks relating to the health, safety and welfare of service users and others who may be at risk.

At the inspection on 23 September 2025, we found that in the following areas the practice was not complying with the relevant regulation. In particular:

A lack of oversight and leadership at the practice continued to contribute to omissions and some ineffective systems.

Improvements were required to the processes in place for identifying and managing risks effectively.

Paper records were not stored securely to comply with General Data Protection Regulations.

Improvements were still underway to ensure that outcomes of complaints and concerns were documented and discussed to share learning and for improvement.

Staff had carried out a disability access audit, dated 26 August 2025, but this was not wholly reflective of the practice and contained the same inaccuracies identified at our previous inspection. For example, it stated that the space was wide enough for wheelchairs, but the practice is not accessible for wheelchair users due to being on the first floor, with multiple steps and narrow corridors. It also referred to a wheelchair lift, when the practice does not have one. Following our feedback, we received evidence that the audit has been reviewed again and amended.

However, we found the practice had made the following improvements.

Improvements had been made to the systems for ensuring that information in the form of policies and procedures were kept up to date and made accessible to staff.

Some improvements had been made to the processes in place for investigating incidents and accidents, and a significant event log had been implemented to ensure these were recorded in the future.
