Well-led – this means we looked for evidence that service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture.

At our last assessment we rated this key question requires improvement. At this assessment the rating has changed to good. This meant the service was consistently managed and well-led. Leaders and the culture they created promoted high-quality, person-centred care.

Shared direction and culture

Score: 3

The provider had a shared vision, strategy and culture. This was based on transparency, equity, equality and human rights, diversity and inclusion, engagement, and understanding challenges and the needs of people and their communities.

During our previous visit in March 2026, we found the provider did not always demonstrate practice which was fully consistent with the principles of ‘right support, right care, right culture’. Unnecessary restrictions were in place within one person’s environment and there was insufficient evidence to demonstrate these had been regularly reviewed, or less restrictive alternatives had been considered.

During our assessment on 6 August 2026, we found the restrictions had been removed and replaced with measures, proportionate to the identified risks. Records demonstrated the person had been involved in decisions about the alternative arrangements, and the provider had considered the least restrictive options available.

These improvements demonstrated a greater commitment to the principles of, ‘right support, right care, right culture,’ ensuring people received support which promoted their rights, independence, choice and quality of life while maintaining their safety.

Governance, management and sustainability

Score: 3

The provider had clear responsibilities, roles, systems of accountability and good governance. They used these to manage and deliver good quality, sustainable care, treatment and support. They acted on the best information about risk, performance and outcomes, and shared this securely with others when appropriate.

During our previous assessment in March 2026, governance systems had not always been effective. Audits had failed to identify and address shortcomings in recording practices. In addition, recommendations arising from fire safety audits and medicine management arrangements had not been effectively monitored, resulting in areas of concern not being identified or acted upon in a timely manner.

During this assessment, we found significant improvements had been made. Risks relating to fire safety and medicine management were being effectively managed, and comprehensive auditing systems were in place. Recent audits had identified areas requiring attention and records showed actions had been taken to address these.

The provider had strengthened oversight and quality assurance processes, which enabled leaders to identify concerns, monitor performance and drive improvements. This demonstrated the provider had established more robust governance arrangements and was using these effectively to support the delivery of safe, high-quality care.

Learning, improvement and innovation

Score: 3

The provider focused on continuous learning, innovation and improvement. They encouraged creative ways of delivering equality of experience, outcome and quality of life for people.

During our previous assessment in March 2026, we found the provider’s understanding and application of the Mental Capacity Act 2005 and Deprivation of Liberty Safeguards (DoLS) had not always been reflected in practice. Restrictive measures had not been fully reviewed to ensure they remained necessary, proportionate and based on individual risks and needs. This limited the provider’s ability to demonstrate they had considered less restrictive alternatives.

During this assessment, we found the provider had acted on the concerns identified at the previous inspection. Records showed restrictive practices had been reviewed in line with the principles of the Mental Capacity Act 2005 and ‘right support, right care, right culture’ guidance. Restrictions which were no longer required had been removed and replaced with measures which were proportionate to the risks identified.

These improvements demonstrated the provider had learned from previous concerns and had used this learning to improve practice. As a result, people were supported in an environment where restrictions were minimised, individual rights were promoted and care was delivered in a manner which balanced safety with independence and choice.
