Governance, management and sustainability

The practice had a governance system that included policies and procedures, which were accessible to staff and were reviewed on a regular basis.

Systems and processes were embedded, and the inspection did not highlight any issues or omissions.

Staff were aware of the importance of protecting patients’ personal information. Staff password protected patients’ electronic care records, and paper records were stored securely and complied with General Data Protection Regulations.

There were effective processes for identifying and managing risks, investigating incidents and accidents, and for receiving and acting on safety alerts.

The practice had systems and processes for learning, quality assurance and continuous improvement. This included undertaking audits according to recognised guidance.

Concerns and complaints were responded to appropriately, and outcomes were discussed to share learning and for improvement.

We noted innovative approaches to providing person-centred care. For example, staff in the practice were oral health educators and went out to local primary schools to give talks on the importance of maintaining oral health and hygiene. Staff also described supporting patients in developing individualised personal anxiety management toolkits.

Staff had clear responsibilities, and systems of accountability to support good governance.

Staff feedback was obtained through meetings, surveys, and informal discussions. They were encouraged to offer suggestions for improvements to the service, and they said these were listened to and acted upon, where appropriate. For example, staff expressed that it would be useful and efficient to have templates when they requested information from external partners, such as other health care professionals. The practice developed templates to accommodate this.

Staff were also able to discuss certain tasks being done on different days as there would be more time to accommodate them on quieter days.

The practice gathered feedback from patients, the public and external partners, and responded accordingly. For example, patients using the accessible waiting area fed back that there was no way to communicate with reception if they had any queries. In response, an intercom system was installed, allowing them to communicate easily with the reception team whenever assistance was required.

Patients also expressed they would like music played in the waiting room. The practice introduced appropriate background music to create a calm and welcoming environment.

The practice had taken steps to improve environmental sustainability. For example, the practice recycled as much as possible. Staff in the practice also turned equipment and lights off when not in use. The practice was working towards paperless correspondence.
