Assessing needs

Score: 3

We did not look at Assessing needs during this assessment. The score for this quality statement is based on the previous rating for Effective.

Delivering evidence-based care and treatment

Score: 3

The service planned and delivered people’s care and treatment with them, including what was important to them. They did this in line with legislation and current evidence-based good practice and standards. A review of clinical searches identified practice in line with national guidance.

The provider had systems in place to ensure staff kept up to date with evidence-based guidance and legislation. As part of our assessment, we conducted a detailed review of people’s care records. These demonstrated that care and treatment were delivered in line with current national guidance and best practice, helping to ensure people received safe and effective care.

Antimicrobial prescribing was monitored through audits, with prescribers supported by local formulary guidance. Audit findings were shared with the wider team to promote learning and improve prescribing practice.

Supporting people to live healthier lives

Score: 3

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

Staff focussed on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity.

The practice actively supported patients to improve and maintain their health through preventative and community-based initiatives. Through social prescribing initiatives, staff made approximately 200 referrals to local support services. These referrals contributed to positive patient outcomes, including supporting a homeless patient to secure stable accommodation.

Monitoring and improving outcomes

Score: 3

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

The service was below national targets for the uptake of childhood immunisations and cervical screening. For example, the number of women in the 25-39 age group that had received a smear was 51.8% for the year 2024/2025. The practice was working to improve scores and had implemented several targeted interventions aimed at improving uptake of screening and immunisation programmes, particularly within the local community. Initiatives included outreach clinics, educational presentations and local newspaper advertising and collaboration with community organisations. The practice was able to demonstrate (with unverified data) that improvements had been made in cervical screening in the year 2025/2026 which had increased to 82% for both age groups.

The practice took part in a study to understand why some children did not receive their vaccinations on time. The practice listened to parents, staff and community partners and reviewed how appointments, reminders and catch-up vaccinations were managed. This helped them to identify any barriers and ways to improve the service. For example, including more flexible appointments and checking the vaccination needs of all children in a family.

The practice was able to demonstrate a proactive approach to quality improvement and population health management. This included the use of risk dashboards to identify patients requiring intervention, regular performance monitoring against national and local quality indicators, and targeted quality improvement projects aimed at reducing health inequalities following a practice merger. The provider also demonstrated improvements in prescribing practice through ongoing de-prescribing initiatives, with evidence showing reductions in prescribing of high-dose opioids and other medicines associated with dependence.

We did not look at Consent to care and treatment during this assessment. The score for this quality statement is based on the previous rating for Effective.
