During an inspection looking at part of the service

At our last inspection April 2013 we found people's wishes were not always considered and their choices not always respected. We also found staff were not always familiar with people's care needs and people's care plans were not updated in a timely manner. We noted there was not a high standard of cleanliness and procedures were not in place to ensure equipment, such as commodes, could be cleaned effectively. We found internal procedures for recording of medicines received into the home were not always followed and there was no clear procedure for the application of 'as needed' medicines.

On this occasion we observed people were treated with respect. People made choices throughout the day and these were respected by staff. We spoke with six of the 14 people living in the home at the time of our inspection. People we spoke with told us staff always treated them with respect. One person said 'it's lovely here. I have no complaints at all'. Care plans reflected people's current needs and were reviewed and updated regularly. The home was clean and effective infection prevention and control procedures were in place. Staff administering medication followed the home's procedures.

The home had a quality assurance policy in place and we saw this was followed. This meant feedback from staff and people using the service was recorded and acted on.

In this report the name of a registered manager, Elizabeth Randall, appears who was not in post and not managing the regulatory activities at this location at the time of the inspection. Their name appears because they were still a Registered Manager on our register at the time of the inspection.

During a routine inspection

During the visit we observed how people spent their time, the support they received from the staff and whether they had positive outcomes. There were twenty two people accommodated at the time of our inspection. We spoke with six people and observed the support other people were receiving from the staff. We found people were treated with respect and the staff and people interacted well with each other. We also spoke with a visitor, a visiting professional and six staff. People told us they were happy with the care they received. They commented the food was 'very good and plenty to eat'. Another person told us they had a 'cooked breakfast when I feel like it'. They told us the 'staff are very friendly'. A person said they were very happy living at Osborne Cottage and said 'it is lovely, if a taxi came to take me home now I wouldn't go'.

We observed the staff were friendly, respectful and courteous when speaking and attending to people. We found there were some care plans in place; however these lacked details and did not always reflect people's current needs. There was a process to manage people's medicines and improvement was needed to ensure people receive their medicines in a consistent way. The health and safety of people and infection control management were not always adequate. There was a process for staff support including supervision and training. This process did not identify training needs and measures to ensure staff practices remained current and up to date.

During an inspection looking at part of the service

We spoke with six people who lived at the home. We also spent some time in the home's communal areas observing the way people were cared for.

Everyone we spoke with confirmed that they were very happy living at the home. People said that they had no concerns about how their care needs were met and that staff knew how to care for them. We were told that staff gave people their medications when they needed them. People said that staff were very friendly and

were available when they needed them.

We observed that people were enjoying their lunch time meal. Those able to respond told us meals were good and that alternatives were provided. People in one part of the home told us how there had been a lot of cleaning completed following our previous visit and that they now had their own cleaner for that part of the home.

People said that if they had any concerns or complaints they would raise these with the manager.

During a routine inspection

We spoke with many people who lived at the home. We also spent some time in the home's main lounge observing people and the way they were cared for. Some people who had Dementia were cared for in a specific area of the home. We spent time in this part of the home, talking with some people and visitors and observing how they were cared for.

We also spoke with other professionals involved in the care of people. They stated that they had no concerns about how people's health and care needs were met. Professionals were complimentary about the way the service met people's physical and cognitive needs.

Everyone we spoke with confirmed that people's privacy and dignity were maintained at all times and that people were able to make day to day decisions such as what time they got up and how and where they spent their time.

We observed that people were enjoying their lunch time meal and those able to respond told us meals were good and that alternatives were provided. People told us that they had gained weight since being at Osborne Cottage. People told us that cooked breakfasts were provided for those who wanted them.

People said that they had no concerns about how their personal care needs were met. They also told us that if they were unwell then staff would contact a doctor for them. We were also told that staff were available when people needed them and knew what care they required.

Visitors said that they felt staff were available whenever their relatives needed assistance. They also said that staff were very pleasant and had the necessary time to meet people's needs.

People and relatives said that if they had any concerns or complaints they would raise these with the manager. Nobody had any concerns when we spoke with them.
