Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence. At our last assessment we rated this key question good. At this assessment the rating has remained good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

Score: 3

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. People's needs were assessed and reviews took place. A member of staff told us, “Care plans were reviewed every month or sooner if there was reason to due to changing needs.”

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards. Where people required assistance, this was provided by staff who sat alongside them and chatted to them. People who required special diets were offered them at mealtimes and where people required additional aids, such as plate guards, these were provided.

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. The staff team worked well together and supported each other to ensure people's needs were met. The nurse liaised with clinical care homes team and was able to answer their questions and share information appropriately. The clinical and care teams knew people they provided care for very well.

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support. Care plans evidenced people had access to health care professionals such as GPs and occupational therapists (OTs). There was provision in place so that if people's condition deteriorated, a member of staff would contact healthcare professionals such as the dietician or tissue viability nurses.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive and consistent and they met both clinical expectations and the expectations of people themselves.  Care plans were reviewed and updated to reflect any changes to care and support. Daily notes and records were completed to monitor people’s health and wellbeing. People had access to local health and care services to support them to manage their wellbeing.

The provider did not always tell people about their rights around consent; however, they did respect people’s rights when delivering care and treatment. Some people’s care plans did not record their agreement to receive care and support but there was clear information from people about their wishes to have their family members involved in decisions about their care which was respected.
