Condition of this registration relating to carrying out this regulated activity
A) The registered provider must not provide personal care to any new service user without the prior written agreement of the Care Quality Commission.
B) The registered provider must carry out audits on all service user care files to ensure systems and processes are effective in identifying and mitigating risks to service users. This must include a full audit on I, II, III and IV. These audits must be shared with CQC within 14 days of this Notice, and every 14 days thereafter. Audits must include:
I) Information on how risk is mitigated for all known risks and individual health conditions
II) Details on how the provider is assured all care plans are factual, up to date and relevant
III) Actions taken and de-escalation techniques used for incidents of distress and assurances that these were appropriate and identified any improvements needed
IV) The review of daily nutrition and fluid intake for service users identified at risk ensuring they are tailored to individuals and records are in place and monitored
C) The registered provider must carry out and send to the Care Quality Commission, written documentation, on how they will implement and embed systems and processes to ensure oversight of medicine management.
This must include a full audit on I, II and III. These audits must be shared with CQC within 14 days of this Notice, and every 14 days thereafter. Audit should
include:
I) Stock checks, signatures, reasons for administering, and correct prescription details are all recorded within MAR chart
II) Records of Staff medicine training and competencies are up to date
III) PRN protocols and emergency medicine plans are in place and regularly reviewed.
D) The registered provider must send to the Care Quality Commission written documentation on how they will implement and embed systems and processes to ensure oversight of safeguarding. This must include a full audit on I, II and III. These audits must be shared with CQC within 14 days of this Notice, and every 14 days thereafter. Audit must include details of:
I) All incidents, accidents and near misses that could put service users at increased risks; such as missed calls, missed or late medicines, falls, behaviors of distress, deterioration of health.
II) How incidents have been reported to relevant professional bodies and how escalation processes are effective in early escalation trigger protocols.
III) How service users have been involved in discussions and processes in relation to mental capacity and consent.
E) The registered provider must carry out audits to ensure systems and processes are effective in identifying concerns and improving outcomes in relation to good governance. This must include a full audit on I, II and III. These audits must be shared with CQC within 14 days of this Notice, and every 14 days thereafter. Audit to include:
I) Details of all staff training, including attendance, any competencies completed and any gaps in staff training needs.
II) timeliness of care calls identifying rationale for any late, early or missed calls. Reviews of suitability of call times, effectiveness of the staffing, and staff tasks
III) Spot checks completed on staff, supervision records, feedback surveys from service users and oversight of safe recruitment.
