During a routine inspection

This inspection took place on the 27 & 28 January 2015. This was an announced inspection which meant the provider knew two days before we would be visiting. This was because the location provides a domiciliary care service. We wanted to make sure the provider would be available to support our inspection, or someone who could act on their behalf.

My Little Angels is registered to provide personal care (not nursing) to adults who may have dementia / physical disabilities. At the time of this inspection 22 people were receiving the service which operates from well-equipped premises.

There is a registered manager in post at My Little Angels. A registered manager is a person who has registered with the Care Quality Commission to manage the service. Like registered providers, they are ‘registered persons’. Registered persons have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated Regulations about how the service is run.

The registered manager was accessible and approachable. Staff, people who used the service and the majority of relatives felt able to speak with the registered manager and provided feedback on the service. However a concern was raised which meant a person was not supported to feel safe when notice was given to them; when the agency had to stop providing care to them.

Staff were knowledgeable of people’s preferences and care needs. People told us the regular staff they had, provided them with the care and support they needed and expected. People using the service, and the relatives we spoke with described the staff as being “very gentle”, “very, very pleasant” and “such nice people”. Staff explained the importance of supporting people to make choices about their daily lives. Where necessary, staff contacted health and social care professionals for guidance and support.

Staff had received training in how to recognise and report abuse. All staff were clear about how to report any concerns they had. Staff were confident that any concerns raised would be fully investigated to ensure people were protected. However the staff we spoke with were less knowledgeable about the requirements of the Mental Capacity Act 2005. Staff said they “felt supported”, and they “received regular supervision.”

Care staff were clear about their roles and responsibilities, and those of the registered manager. However non care staff working for the agency were not clear of theirs. There was no defined company structure and this was causing confusion and an error in responding to a complaint according to the company policy. We discussed this with the registered manager, who agreed to address this immediately.

The registered manager had systems in place to monitor the quality of the service provided. Staff were aware of the organisation’s visions and values and spoke about being ‘proud’ to work for My Little Angels.

During a routine inspection

At the time of our inspection eight people were receiving care and support from My Little Angels.

We spent part of one morning visiting a person in their own home. We spent time in the office reviewing records and talking with staff. We spoke on the telephone with three people who use My Little Angels, two relatives and two staff. A person said: "I am very pleased with them. I cannot fault them at all. They are extremely reliable and caring." Another person, supported by the service said: "I think it's exceptionally good." A relative told us "I have complete confidence in the staff who are compassionate and professional. My relative has all of her care and social needs tended too and never wants for anything.'

Staff were able to tell us with confidence about treating people they supported with consideration and respect.

The provider used their knowledge, experience and systems to ensure staff were able to keep to their agreed visits to people. This involved having sufficient staff, scheduling visits to people carefully, and factoring in planned and unplanned staff absence.

We found the management of people's medicines was well organised and staff were trained effectively to carry this out safely.

We found the organisation had effective recruitment practices to assure staff employed were able to work with people safely and effectively. One person told us 'I receive support from staff who know me very well. They provide care and support which is consistent and what I need and expect. When a new member of staff is employed, they are always introduced to me first by the provider. Then they are shadowed until they know what is expected of them. The provider runs a very tight ship and has high expectations, they always recruit a high calibre of staff who have high standards themselves. Another person told us 'the care and support I receive is second to none. They have years of experience and receive regular training.'

Everyone we spoke with confirmed they were very happy with the service My Little Angels provided. The agency had systems in place to monitor and assess the quality of its service and showed examples of improvements they had made to service delivery in response to comments people who used the service.

During a routine inspection

As part of this inspection, shortly after our visit we contacted one relative and one person who uses 'My Little Angels'. We also spoke with the provider.

Everyone we spoke was complimentary of the staff and were very satisfied with the care and support provided. Comments included: "I cannot fault the care and attention I receive. The staff go over and beyond the call of duty.' 'The small staff team are extremely caring and experienced and know me very well, they recognise if I am not feeling well and take action.' 'The staff ensure that I am in total control of my care and support and I have plenty of opportunities to express my views regarding the care or treatment I receive.' ' The staff are very flexible and ensure their visits fit in with me to meet my individual needs.' 'The staff are well trained and are thorough in the support they provide. They ensure I am comfortable and have everything I need before they leave.' 'The staff encourage me to do as much for myself as possible and this enables me to stay in my own home, and be as independent as possible.' 'Staff are very aware of the importance of privacy and dignity, they are polite and respectful. New staff have been introduced and work alongside the provider gradually until I have felt confident that the new staff knows me and my routine, which I value.'
