Supporting people to live healthier lives
The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. The practice supported people to live healthier lives and where possible, reduce their future needs for care and support.
Staff focussed on identifying risks to people’s health. There were monthly meetings where vulnerable patients, who were frail, housebound, receiving end of life or palliative care were discussed and supported. Those with caring responsibilities were identified and offered health checks and support. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity.
Monitoring and improving outcomes
The practice routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.
The provider met national targets for the uptake of childhood immunisations. Three out of five of the vaccines given were 100% achieved. The cervical screening figures for those aged 50-64 were above the England average, with those aged 24-49 3% lower than the England average.
