Background to this inspection

BeCC:Bespoke Complex Care Support provides community healthcare treatment to vulnerable children and young people in their homes. The children and young people may have life limiting conditions or physical healthcare needs that need at home nursing support, for example to care for tracheostomies.

The service has an office base in Ripley, Derbyshire but delivers care in the homes of children and young people in their homes in West Yorkshire (Bradford, Craven, Airedale and Leeds). At the time of our inspection the service was providing support to 30 young people and children.

The service works closely with other care professionals involved in the care of the child or young person. They can provide care 24 hours a days 7 days weeks usually in a young persons home and have an out of office service for our staff and families to use should they need to seek help outside of office hours. They aim is to support children and young people to maintain a family life when otherwise their care needs would require hospital admission.

The service is registered to provide the following regulated activities:

  • Diagnostic and Screening procedures
  • Treatment of Disease, Disorder or Injury

There was a registered manager at the time of this inspection. This was the first inspection of this service since 2013 and as such there were no previous ratings.

What people who use the service say

We spoke with four clients who were all very positive about the care they were being provided. They said that staff were caring and supportive. One client said that sometimes staff did not let them know when an appointment had been cancelled until too late. All clients knew how to raise concerns with the provider and felt comfortable to do so.

We rated this service as good:

  • The service had enough staff to care for children and young people and keep them safe. Staff had training in key skills, understood how to protect children and young people from abuse, and managed safety well. The service controlled infection risk well. Staff assessed risks to children and young people, acted on them and kept good care records. They managed medicines well. The service managed safety incidents well and learned lessons from them.
  • Staff provided good care and treatment, gave children and young people enough to eat and drink, and gave them pain relief when they needed it. Managers monitored the effectiveness of the service and made sure staff were competent. Staff worked well together for the benefit of children and young people, advised them and their families on how to lead healthier lives, supported them to make decisions about their care, and had access to good information. Key services were available seven days a week.
  • Staff treated children and young people with compassion and kindness, respected their privacy and dignity, took account of their individual needs, and helped them understand their conditions. They provided emotional support to children and young people, families and carers.
  • The service planned care to meet the needs of local people, took account of children and young people’s individual needs, and made it easy for people to give feedback. People could access the service when they needed it and did not have to wait too long for treatment.
  • Staff understood the service’s vision and values, and how to apply them in their work. Staff felt respected, supported and valued. They were focused on the needs of children and young people receiving care. Staff were clear about their roles and accountabilities. The service engaged well with children, young people and the community to plan and manage services and all staff were committed to improving services continually.

However:

  • Not all staff were receiving supervision in line with the providers policy and no staff had received a recent appraisal.
  • Risks relating to the service were not kept updated on the organization's risk register. This meant it was not possible to see how they were being addressed.
  • Some systems and governance were still being embedded following a number of changes of provider in the year before this inspection
