Supporting people to live healthier lives
The practice supported patients to manage their health and wellbeing to maximise their independence, choice, and control. The practice supported patients to live healthier lives and where possible, reduce their future needs for care and support. Staff could signpost and refer patients to external services and support groups. Information on different services was also available to patients on the practice website.
Staff focussed on identifying risks to patient’s health, including those in the last 12 months of their lives and those with caring responsibilities. The practice was aware of and engaged with national priorities to improve population health, including prevention, early identification of disease, and reducing health inequalities through promoting uptake of screening, immunisation, and healthy lifestyle interventions.
Monitoring and improving outcomes
The practice monitored all patient’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent, and fully met the expectations of patients themselves.

The practice met national targets for the uptake of childhood immunisations. The practice’s performance in cancer screening programmes, including breast, and bowel, was in line with England averages. They explained that patients could be booked in for screening or vaccinations opportunistically and had a regular dialogue with their patients so any concerns they had could be discussed.
