During an inspection looking at part of the service

Since our last inspection in December 2012 the provider told us that he had suspended clinics until 20 March 2013. He intended to make improvements to the service. At this inspection, although a clinic was planned for the following day the manager told us the clinic would not operate until he had recruited a doctor.

We checked to see if improvements had been made following our warning notices issued in January 2013 after our inspection in December 2012. The warning notices addressed our concerns about infection control and quality assurance. We checked that action had been taken and systems were in place for when the service recommenced.

We found insufficient action had taken place to ensure that risks in respect of infection control had been carried out. Some changes had been made to the environment by ensuring that hand washing facilities were in place, some soft furnishings had been removed and some cleaning equipment had been provided. Some policies were in place but not for all procedures. There was a lack of risk management, of cleaning schedules and plans to monitor them and ensure that risk of cross infection were minimised. The provider's arrangements for monitoring the quality and risks within the service were not comprehensive and therefore they had not put systems in place which ensured that required standards would be met when clinics were held.

During a routine inspection

We found that there was little information available about the services provided at the clinic for prospective patients. Once at the clinic little information was available about the consultation and options with no leaflets or information about other contacts and or useful information. There was no information about how infection control was managed, about safeguarding or about the range of services on offer or help lines. Records of consultations were limited. A standard form was used which the manager said was signed before seeing the doctor and did not therefore accurately record what people consented to, and there was no record of discussions about liaison with GPs. This meant that the service was provided in isolation and relied on patients self reporting.

We found that the management systems were not developed with no evidence of quality monitoring or assessment of risks within the service. We found errors in medicine management, reporting to the commission, and no statement of purpose. We found no infection control audits, no safeguarding procedures and inadequate recruitment procedures.

We asked the service to contact fifteen people and were able to talk with two who were positive about the service they received. However we found that the service had no formal system of obtaining feedback from patients.
