At our last inspection this key questions was rated good. At this inspection this key question has remained good. This meant people’s needs were met through good organisation and delivery.

Person-centred Care

Score: 3

The provider made sure people were at the centre of their care and treatment choices and they decided, in partnership with people, how to respond to any relevant changes in people’s needs. Staff understood people well and adapted their approach based on individual preferences.

People said staff took time to ensure they felt comfortable. Despite some minor updates needed to care plans, practice was consistently person‑centred.

Care provision, Integration and continuity

The provider understood the diverse health and care needs of people and their local communities, so care was joined-up, flexible and supported choice and continuity. Care was coordinated and consistent.

Staff used handovers and electronic records effectively. MDT working was strong and supported safe, joined‑up care. Some minor documentation gaps existed but had not led to disruptions in care delivery.

Providing Information

The provider supplied appropriate, accurate and up-to-date information in formats that were tailored to individual needs.

Picture menus and clear explanations helped people make choices. Relatives said staff communicated openly and kept them informed.

Listening to and involving people

Score: 3

The provider made it easy for people to share feedback and ideas, or raise complaints about their care, treatment and support. Staff involved people in decisions about their care and told them what had changed as a result.

For example, issues raised in residents’ meetings, such as laundry and meal choices, had been acted on. Relatives expressed very high satisfaction with involvement, and any low level concerns had been dealt with.

Equity in access

The provider made sure that people could access the care, support and treatment they needed when they needed it. People were supported promptly and referred to agencies such as district nurses, Speech and Language Teams (SALT) and psychiatrists in a timely manner.

Equity in experiences and outcomes

Staff and leaders actively listened to information about people who are most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this. Care notes included dates and times when staff followed up on appointments for people along with any follow up actions or recommendations.

Planning for the future

Score: 2

People were not always supported to plan for important life changes, so they could have enough time to make informed decisions about their future, including at the end of their life.

The registered manager supported people and arranged sensitive end of life care, however this was not always consistently recorded in people’s care plans. We raised this at the time with the registered manager who has since taken action to review people’s end of life wishes.
