During a routine inspection

The main purpose of Caraston Hall Support Ltd, is to provide supported accommodation. This service does not require to be registered with the Commission. At the time of this inspection there was just one person living in supported accommodation who also received a personal care service provided by Caraston Hall. Our inspection focussed on the personal care received by this person.

During this inspection we spoke with the operational manager, the registered manager, three care workers, and we also spent time observing the person who received the domiciliary care service and their interaction with a care worker who was supporting them. The person had no verbal communication skills but we saw they were happy and relaxed.

We looked at their care plan, assessment records, risk assessments, and records of the care provided to them each day. Care plan files contained sufficient information about the person's needs and any risks associated with their care and support.

We also looked at records relating to supporting staff, and assessing and monitoring the quality of services. We found that the provider had carried out a range of actions to improve the quality of the services provided to people. Staff we spoke with told us they were well supported, and had received a good range of training. Comments from staff included 'People are now getting the support they need' and 'This is a really good place for people to live'.

During a routine inspection

People receiving the domiciliary care service also received continued support within a supported living framework from the same staff who provided personal care. Staff told us about common concerns about how the service was supporting the staff who provided personal care such as staff training, hours, communication between staff and management, on-call arrangements and safety of staff when carrying out care and support for people who had behaviour which could be challenging for staff. We had received information before this inspection raising concerns about staff support which had also been raised with the local safeguarding team. We informed the local Health and Safety Executive (HSE) with responsibility for health and safety at work about some of these concerns.

However, people's care was personalised and there was good information to enable staff to meet people's personal care needs and involve people. We saw staff had a good rapport with people receiving the service and had knowledge of their individual complex care needs. People's independence was promoted and as personal care was delivered within a supported living setting, this meant that support could be given in a very flexible way. For example, support with showering depended on when the person wished to shower rather than at a set time. We saw that staff were available to see people using the service if they needed assistance or had any concerns as there was an agency/supported living office on the premises.
