Person-centred Care

The provider was exceptional at making sure people were at the centre of their care and treatment choices and they decided, in partnership with people, how to respond to any relevant changes in people’s needs.

There was a strong focus on promoting independence, with people supported to regain and maintain everyday skills and confidence.

Care was highly flexible and responsive, enabling people to continue doing things that mattered to them, including visiting places of interest and spending time with people important to them, without unnecessary restrictions such as call times not being able to be changed.

The provider promoted people's wellbeing, social inclusion and quality of life. People were supported to take part in meaningful activities tailored to their interests and preferences and were encouraged to remain active members of their local communities. The provider created opportunities for people to develop relationships, reduce social isolation and participate in social events, including seasonal celebrations and events recognising the achievements of the service. The service offered personalised activities and craft opportunities that reflected each person's interests, resulting in exceptional levels of engagement, wellbeing, and social inclusion This person-centred approach empowered people to live fulfilling lives and achieve outcomes that were important to them
